Job summary
We are seeking a motivated and experienced
Paramedic to join our multidisciplinary team. The successful candidate will
work across five practices, conducting home visits and supporting clinical
teams with a wide range of paramedic duties. You will play a vital role in
improving access to care, reducing pressure on GPs, and enhancing patient
outcomes.
Main duties of the job
Home Visits: Conduct clinical
assessments and treatments in patients homes, particularly for housebound and
vulnerable individuals.
Minor Illness Clinics: Diagnose
and manage minor illnesses and injuries independently or with GP support.
Triage and Assessment: Support
telephone and face-to-face triage, assessing patients and directing them to
appropriate care pathways.
Chronic Disease Management:
Assist in monitoring and managing long-term conditions such as asthma, COPD,
diabetes, and hypertension.
Emergency Response: Provide
urgent care and support in emergency situations within the practice setting.
Medication Reviews: Conduct
reviews and support prescribing within scope of practice.
Health Promotion: Educate
patients on lifestyle, prevention, and self-care strategies.
Documentation: Maintain
accurate and timely clinical records using SystmOne or other clinical systems.
Collaboration: Work closely
with GPs, nurses, pharmacists, and other allied health professionals to ensure
coordinated care.
Safeguarding: Recognise and act
on safeguarding concerns in line with local and national policies.

Qualifications
Essential
- HCPC registered Paramedic
- Experience in primary care or community settings
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal abilities
- Ability to work independently and across multiple sites
Desirable
- Independent Prescriber qualification (or working towards)
- Experience with SystmOne or similar clinical systems
- Additional qualifications in chronic disease management, minor illness, or advanced clinical practice
