PCN Clinical Pharmacist

Job summary

Oldham Central PCN is seeking a Clinical Pharmacist to join our well established PCN pharmacy team. This is an exciting opportunity to join an innovative and supportive multi-disciplinary team. The post holder will be a pharmacist, who acts within their professional boundaries, providing clinical support within the PCN and working as part of multi-disciplinary teams in general practice. In this role you will receive appropriate clinical supervision. You will ensure practices integrate with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare and help manage workload. Perform a patient facing role to include undertaking clinical medication reviews to proactively manage patients with complex polypharmacy in a variety of settings including the surgery, patient homes and residential care, and with a view to meeting demands of new workstreams in primary care. Provide support to general practice staff with regards to prescription and medication queries, providing expertise in clinical medicines advice.

You will be supported to develop your role to become a non-medical prescriber.

Main duties of the job

You will be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) for encouraging the development of better understanding of the principles of medicines optimisation throughout the PCN and practice teams, promoting good practice in line with national requirements, NICE implementation and utilisation of.

Action clinical letters and discharge summaries, providing leadership on person centred medicine optimisation and quality improvement. Assist with drug monitoring including high risk drug monitoring, switching medications to agreed and approved protocols, Manage the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions & medicines reaching review dates, minimising clinical risk and aiming to reduce wasted medicines.

Clinical Pharmacy in General Practice is an integral part of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the PCN. The Clinical Pharmacy in General Practice model is supported by the direction of national policy, where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.

Clinical Pharmacist play a key role, complementing and working alongside the PCN pharmacy technicians and other members of the PCN multi-disciplinary team.

Ideally, the post holder will be a prescriber, or willing to complete training to become a prescriber. Qualifications

Essential

  • Accredited masters degree of pharmacy Degree (MPharm) (or equivalent)
  • Registered with General Pharmaceutical Council (GPhC)
  • Completed, or working towards, a Primary Care Pharmacy Education pathway qualification (CPPE)
  • Minimum of 2 years post qualification experience

Desirable

  • Membership of the Royal Pharmaceutical Society
  • Independent prescriber or working towards/intent of gaining an independent prescribing qualification

  • Minimum of 2 years post qualification experience
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Able to obtain and analyse complex technical information
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
  • Can work independently on a day-to-day basis and able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long-term conditions
  • Excellent written and verbal communication skills (Email, telephone & online meetings)
  • Evidence of consistent pattern of learning from education, training, and experience
  • Able to work under pressure and to meet deadlines
  • Good IT skills
