Job summary
We are looking for an organised, compassionate, and proactive Care Coordinator to join our team. In this role, you will play a key part in supporting individuals to access the right services at the right time, ensuring their care is well-planned, person-centred, and responsive to their needs. The role will primarily focus on triaging GP online consultation and telephone triage requests, supporting the clinical team through prescribing clerk duties, and assisting with wider administrative and reception responsibilities.
Main duties of the job
Administration duties include scanning and filing incoming post as
well as electronic clinical letters.
Deal with incoming practice email queries directly from
hospitals or care agencies.
Assist with signposting patients to appropriate community or
local services, alongside social prescribers.
Act on communications from hospitals or community providers.
Action and follow death protocol procedure allowing a smooth
process for families or close relatives.
Manage incoming medical reports and SAR requests (medical
record requests) from patients and third parties.
Assist with and ensure patients participate in local or
national screening programmes.
Pro-actively ensure patients participate in vaccination
opportunities such as flu, shingles, or childhood immunisations.
Organise and prioritise own workload.
This is a patient-facing non-clinical role that requires
excellent organisational skills, attention to detail, and a good understanding
of General Practice processes. Experience of working in a GP surgery and
experience with clinical triage systems is desirable.

- Liaise with patients, pharmacies, and clinicians regarding prescription queries.
