Job Summary
The Digital & Data Analyst
needs to be passionate about using digital channels and is responsible for
internal communications and employee engagement programmes, press and public
affairs and corporate wide creative projects. This role will suit someone who
is interested in joining a team working to drive change in our communications
approach to engage, inform, and inspire our audiences. They will perform a
range of administrative tasks and the role involves a significant amount of
communication with patients, their family members and various Healthcare
Professionals.
The successful applicant will be
required to have strong IT skills and attention to detail for the data
collection. The data provided to the PCN team will be used to contribute to
written reports by the Primary Care Network Operations Manager. Additional
training can be given on the clinical audit work.
The post holder will also, as
required, provide data collection to help the wider PCN team achieve other key
quality improvement, patient safety initiatives and work programmes.
Main Duties and Responsibilities
To obtain data and information for
the PCN to:
Carry out Feedback via Google Forms on PCN service
Increase
the capacity and access levels for the PCN, and its member practices.
Utilise
SystmOne Clinical Reporting to identify cohorts of patients that will help
the PCN with Health Inequality identification
Fully
utilise Systmone for the PCN to increase coverage of medical file records
(Such as Blood pressures, Smoking status)
Work
with the Operations Manager to provide digital maturity to the PCN
Collect
and collate data for activities such as the monthly data sheet that goes
to practices and more
Create
and manage the PCN and member practices social media accounts, in line
with the NHS Campaigns and PCN work
Create
posters for events and services offered by the PCN
Work
with the Management Team on creating Health & Wellbeing Days in line
with Integrated Neighbourhood Frameworks.
Create a
newsletter bi-monthly that show cases PCN work and upcoming events within
the PCN
Update
the website and digital screens with PCN/Practice information and upcoming
events.
Utilise
Windows based applications such as MS office (Powerpoint, Excel, Word)
Contribute
as a team member to supporting other staff employed in the PCN to carry
out the delivery of the objectives using data.
The Digital
& Data Officer will work with the Primary Care Network Operations
Manager/Clinical Director, Public Health Teams and CCG to obtain the PCN
baseline measures for the identified specialties.
Attend
monthly MDT meetings on behalf of the surgeries to discuss and coordinate
care for vulnerable patients.
Be able
to fully utilise the wider PCN team, and have good pathways to book in
with a wide range of healthcare professionals
Data Collection
Set up
and run SystmOne searches, under the instruction of the Primary Care
Network Operations Manager, Public Health and the Clinical Directors, to
identify the PCN needs and also to monitor progress against indicators.
Collect data
and review the digital triage system in accordance with PCN needs
Retrieve
data from information systems and clinical documentation for the purpose
of undertaking clinical audit (consistent with the audit measures and data
collection instructions) or support clinical staff carrying out these
functions directly.
Set up
databases and spreadsheets for the collection of audit data and enter
audit data as required
Check
the accuracy, completeness and timeliness of data entered in to a database
or spreadsheet for a clinical audit and advise relevant people of missing
or inaccurate data or an impending deadline for submission of complete and
accurate data.
Analyse
or assist clinical staff in analysing audit data appropriately so that the
level of compliance with best practice can be determined and the results
used to prompt changes in practice as necessary.
Contributing to carrying out work
needed to ensure that the PCN meets external requirements related to clinical
audit.
Maintain
documentation related to clinical audit consistent with approved policies,
the Data Protection Action, General Protection Data Regulations (GDPR) and
Human Rights Act and ensure that the information is available for
reporting purposes.
Record
progress of clinical audit activity on the PCN database systems ensuring
that there is a detailed, timely and accurate record of all allocated
projects. This will include evidence of improvements achieved through
clinical audit.
Work
with the PCN operations manager for monthly and quarterly claims, relating
to the DES or other NHS initiatives (such as the Winter Access Fund)
Contribute to supporting the
smooth running of the Primary Care Network.
Attend
meetings as required and any other local or regional audit meetings
pertaining to Capacity and Access as requested by the PCN Manager or
Clinical Director.
Attend
meetings with the other PCN Clinical Audit Team for the purpose of
reviewing the departmental activity and sharing general information.

Service improvement
Work with colleagues in the team on the development of
current and new services and other initiatives;
Work closely with the whole PCN team at Southend East PCN.
Quality
Alert other team members to issues of quality and risk
in the care of patients;
Ensure own actions are consistent with clinical
governance systems;
Know the organisations policies, especially the
whistle-blowing policy, available in the practice staff handbook;

fulfil the duties of this post;

procedural guidelines.
