Job summary

Here in Penwith Primary Care Network (PCN) we are creating a brand-new model as part of the transformation of mental health care in Cornwall. There has never been a more exciting time to work in mental health services, especially in primary care. We want to hear from people who have passion and creativity to help shape and build our new, holistic service in Penzance, Hayle and St Ives.

We aim to create a space where we understand what matters to you, not whats the matter with you. Our new team will include a diverse range of skills, to listen and support people who are struggling with their mental wellbeing. We are in the process of creating a welcoming hub in Penzance initially, with new hubs across Penwith in the next few months.

We are looking for health and wellbeing coaches to help us realise our ambition. You will be supported by team lead and mental health practitioners. We offer competitive benefits and a happy, compassionate work environment.

This role will be predominantly hub-based, initially in Penzance, with some home/virtual working. The hub will provide booked appointments and, ultimately, a walk-in service.

Main duties of the job

Key responsibilities

Manage and prioritise a caseload, taking a non-judgemental approach based on strong communication and negotiation skills, while considering the whole person when addressing existing issues

Use IT and MDT channels to screen patients, with an aim to identify those who would most benefit from health coaching

Provide personalised support to individuals, their families and cares, to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently

Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their health and wellbeing and subsequently supporting them in shared decision-making conversations

Utilise health coaching skills to support and develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers

Explore and support patient access to a personal health budget, where appropriate, for their care and support

Refer people to other health professionals within the PCN as appropriate

Adheres to a code of ethics and conduct in line with the NHS England health coaching Implementation and Quality Summary Guide.

About us

Penwith Primary Care Network (PCN) includes 7 GP practices and our geography is the far West of Cornwall, including Penzance, St Ives, Hayle, Marazion and St Just. Our legal entity, the employing organisation, is Health & Wellbeing West Limited, a limited company whose directors are from the GP practices. We currently have 21 staff employed under the Additional Roles Reimbursement Scheme (ARRS) and we are looking to recruit additional roles this year.

This is an exciting time in our PCN. Innovation, creative problem-solving and strong links to our local communities are the bedrock of our PCN and we want to build on that to reach more people, in new ways.

We are growing a diverse team of people who are out-of-the-box thinkers, with a wide range of skills, who can change the conversation and provide truly personalised care.

Working with partners in local healthcare and the voluntary sector, we are developing new services and there are multiple opportunities to join us and be part of the adventure. We are recruiting paramedics, advanced clinical nurse practitioners, health & wellbeing coaches, clinical pharmacists, and care co-ordinators. o Increasing patient motivation to self- manage and adopt health behaviours

Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their health and wellbeing and subsequently supporting them in shared decision-making conversations

Utilise health coaching skills to support and develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers

Develop collaborative relationships and work in partnership with health, social care and community and voluntary sector providers and MDTs to holistically support patients wider health and wellbeing, public health and contributing to the reduction of health inequalities

Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input

Raise awareness within the PCN of shared decision-making and decision support tools

Work closely in partnership with the social prescribing link workers, to work alongside people who may need additional support, but are not yet ready to benefit fully from social prescribing

Maintain and update care records, including on GP practice systems.

  • Flexible team player who is equally happy working alone or as part of a team
  • Ability to work autonomously and make decisions within the boundaries and scope of the role
  • Willingness to undertake mandatory training online or face-to-face, and to undertake further training as the service/need develops
