Job summary
We require someone who is experienced in document coding and care navigation, to join our well established admin and reception teams. The post is for 12 months initially but has the potential to become a permanent role. The successful candidate will assist the IT Admin Manager in their duties, including coding hospital and other clinical documents, as well as performing Care Navigation duties.
If you only have skills in either coding or Care Navigation you should still apply. Previous applicants need not apply.
Knowledge of EMIS call and recall is useful but not essential as training will be given.
We will consider job share for the right candidate and we offer an element of home working too, where suitable
Main duties of the job
Coding hospital, clinics and out of hours correspondence on to patient records. Process discharge summaries and book patients follow up appointments with the necessary clinician or create a diary reminder for patients requiring blood tests. Process and create tasks internally to the team. Participate in call and recall of patients for annual chronic disease reviews, this is not an exhaustive list.
Care Navigation will involve signposting patients to the appropriate services, dealing with patient queries, call handling, front desk reception, scanning of documents, repeat prescription processing, processing new patient registrations, this is not an exhaustive list.

Oak Vale Medical Centre has been a long, established GP practice providing medical care for generations of families for more than 90 years and pride ourselves on being patient-centred. We are committed to the education of future doctors and have been a training practice for several decades, with more than 12,500 patients and the list is growing all the time. All salaried doctors were our registrars and returned as GPs, once they finished their training.
We have a full compliment of GPs, Nurses, HCA's as well as junior doctors and in-house Network staff such as, an ANP, Pharmacist and Physiotherapist.
Training is important to us. We have employed apprentices for more than10 years now and most go on to have permanent positions within the practice.
There are separate admin and reception teams, we find this works well.
We are high achievers for QOF and are part of a successful Primary Care Network called iGPC.
