This
is an exciting opportunity to work in a progressive, multi-disciplinary setting
where you can take ownership of medicines optimisation and patient safety, take
the lead in one or more clinical areas and help to ease GP workload and improve
patient outcomes.
This
role replaces our current Clinical Pharmacist who is retiring.
Main duties of the job
To be responsible for medicines management within the
practice and manage the team responsible for delivery.
To be part of the practice management team.
To be responsible for medicines safety within the practice.
To advise on appropriate prescribing in general practice and
provide education on appropriate new medication and guidelines.
To lead on delivering the medicines element of the Local
Incentive Scheme. This will include delivering Structured Medication Reviews
for older patients with polypharmacy.
Assist with QOF where appropriate.
To provide face to face and telephone appointments for
medication review.
To provide face to face and telephone appointments for
chronic disease management as appropriate.
To take the lead for the practice in one or more clinical
areas.
Liaise with local Community Pharmacies to encourage
collaborative working.

Job responsibilities
Medicines Management
To be the practice lead for medicines management and line
manager for the team responsible for delivery of medicines management. The
medicines management team currently consists of one pharmacist and five
administration staff. This role includes being part of the practice management
team.
Clinical Medication Review
Undertake
clinical medication reviews with patients and produce recommendations for practice
clinicians with regard to prescribing and monitoring for patients including
Care Home Patients. These reviews may be face to face, telephone or by text.
Medicine information to practice staff and patients
Respond
to medicine related enquiries from clinicians, other practice staff, other
healthcare teams (e.g. community pharmacy) and patients/carers with queries
about medicines. Suggesting and recommending solutions. Providing follow up for
patients to monitor the effect of any changes.
Management of Medicines at Discharge from Hospital
To
ensure this process is robust
Risk Stratification
Identification
of patients at risk of harm from medicines. This might include risks that are
patient related, medicine related, or both. This will include monitoring of
high risk drugs and shared care.
Medicines Quality Improvement
Undertake
audits of prescribing as necessary, feedback the results and implement changes
in conjunction with the practice team.
Medicines Safety
Implement
changes that result from MHRA alerts, product withdrawal and other local and
national guidance.
Implementation of Local/National Guidelines and Formulary
Recommendations
As
appropriate
Cost Effectiveness
Support
the delivery of QOF and other incentive schemes.
Care Quality Commission
Work
with the general practice team to ensure the practice is compliant with CQC standards.
Public Health
To
support public health campaigns and provide specialist knowledge as
appropriate.
Working Relationships
Recognises
the roles of other colleagues within the organisation and their role to patient
care, whilst working as part of the surgery.
Should
have excellent communication skills.
Actively
work toward developing and maintaining effective working relationships both
within and outside the practice and locality
Patient Facing Long-Term Condition Clinics
See
patients with single or multiple medical problems where medicine optimisation
is required. Review the ongoing medicine plan for these patients, ensuring they
get the best use of their medicines (i.e. medicines optimisation) and make
appropriate recommendations to clinicians for medicine improvement.
Take
the lead for managing and prescribing for one or more long term conditions.
The
NHS changes and as it does members of the team need to change so a successful
candidate will need to be open to a changing job role.
