Job summary

We have an exciting opportunity for qualified and experienced Nurse to come and work for the Kenyon Medical Centres providing high quality patient focused health care in the Coventry.

We are looking for a enthusiastic and committed Practice Nurse to join our team and support the delivery of our excellent patient health care service and in return we will support you in continuing your professional development whilst you work for us.

Main duties of the job

  • Carry out Immunisations, and vaccines.
  • Perform electrocardiography.
  • Carry out ear examinations and syringing if necessary, following appropriate training and assessment.
  • Perform cervical screening or willingness to undertake training.
  • Administer adult and paediatric injections under an individual prescription or PGD ensuring safe storage, rotation and disposal of vaccine and equipment.
  • To administer and check coils or willingness to undertake training.
  • Deliver travel health care including vaccinations and advice for patients following appropriate training if required.
  • Undertake first aid and management of emergencies (e.g., burns asthmas attacks.)
  • To provide flexible cover for members of the Nursing team during periods of sickness and apply Practice policies, standards, and guidance
  • Maintain accurate patient records.
  • Provide professional advice/support to health care professionals within the primary care team.
  • Provide mentorship to the nursing team.
  • Advise and support other team members of the team aiming towards common objectives.
  • Promote your own continuing professional development.

Job description

Job responsibilities

Primary Responsibilities

The following are the core responsibilities of the Practice Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Support the clinical team in the development of individual treatment plans for chronic disease patients.

b. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.

c. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.

d. Provide wound care (ulcer /Doppler etc.) to patients.

e. Provide travel medicine services.

f. Request pathology services as necessary.

g. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.

h. Maintain accurate clinical records in conjunction with extant legislation.

i. Maintain chronic disease registers.

j. Develop, implement and embed well woman clinics.

k. Chaperone patients where necessary.

l. Prioritise health issues and intervene appropriately.

m. Support the team in dealing with clinical emergencies.

n. Recognise, assess and refer patients presenting with mental health needs.

o. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways.

p. Implement vaccination programmes for adults and children.

q. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.).

r. Support the clinical team with all safeguarding matters, in accordance with local and national policies.

s. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

t. Deliver opportunistic health promotion where appropriate.

Secondary Responsibilities

In addition to the primary responsibilities, the nurse practitioner may be requested to:

a. Monitor and ensure the safe storage, rotation, and disposal of medications.

b. Contribute to the implementation of an effective training programme for staff and students within the practice.

c. Provide clinical supervision for the HCA team as appropriate.

d. Participate in local initiatives to enhance service delivery and patient care.

e. Support and participate in shared learning within the practice.

f. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA).

g. Maintain professional registration and adhere to revalidation requirements
