Job summary

The care co-ordinator is the lynchpin of the PCN, creating and sustaining connections across and between multiple health and social care professionals, community services and patients.

Main duties of the job

The key focus for our care coordinators are personalised care for every patient and helping reducing health inequalities in the community.

About us

North Lewisham Primary Care Network (NLPCN) is one of the largest PCNs in Lewisham and has a very diverse population made up of 9 GP Practices serving a population of around 80,000 patients.

Health inequality impacts our patients: North Lewisham has higher than average levels of deprivation and BAME populations and poorer health outcomes.

NLPCN is committed to tackling health inequality as a priority. We have developed a program of work to improve trust and engage with our local community, training our GP surgeries and staff to improve access for patients, improving our communication to patients about service changes and health promotion and improving the monitoring of those suffering from health inequality to drive our strategy.

Job responsibilities

Main Role Responsibilities

Assist patients in navigating the NHS services and products, including showing patients how to use Apps and websites to book appointments, request for medication or access information about health or health services

Help in the planning and arranging of patient care and providing support to patients such as support with filling in applications, reading/understanding literature

Support GP practices by conducting basic health services such as flu/covid-19 vaccinations and health checks

Conduct patient assessments and where required, signpost patients to the appropriate service

Supporting the implementation of PCN, local and national policies such as those aimed at reducing health inequalities

Attend multi-disciplinary team (MDT) meetings with other personalised care roles in the primary care network

Assist/facilitate new PCN projects, activities and clinics

Assist in improving patient access to health services and care e.g. assisting the digitally excluded to learn and access digital health services and tools

Advocating for patients, assist patients in communication or engagement with other health professionals
