Job summary

NLPCN operates as part of the local accountable integrated care system, working and collaborating with GP practices, a wide range of healthcare professionals, social services professionals as well as local community groups. This enables us to not only focus on direct health services but also on the social and other issues that impact on health, thereby enhancing the health and wellbeing of patients and community.

The Care Navigator/ co-ordinator is the lynchpin of the PCN, creating and sustaining connections across and between multiple health and social care professionals, community services and patients.

Job responsibilities

Main Role Responsibilities

To act as a care coordinator and navigator for users of the Waldron Health Centre and those using the services of the North Lewisham PCN

Signposting - support users of the Waldron Health Centre to navigate and access services and to have a clear understanding of the services that are represented

To develop and maintain a directory of services for the Waldron Health Centre as well as other key communities, institutions or services important for the health and wellbeing of the patients.

Understand and be able to communicate the processes and procedures of services in the Waldron in order to advise and help services best navigate care

Carry out administrative/receptionist duties for the North Lewisham PCN including booking in follow-up appointments and or referrals for patients

Facilitate the use of the Waldron as a community space with the Social Prescribing Community Link Worker coordinating and supporting community organizations to use the Waldron in line with the PCN priorities for reducing health inequalities

Work closely with the North Lewisham Community Forum to understand the communities priorities for the health and wellbeing of its residents and promote the Waldron as a community hub

Support service users who struggle to access services for a number of reasons facilitating digital inclusion, language support and working with advocates to help patients navigate care in the Waldron effectively

Develop relationship with Tenant Liaison Manager who will share knowledge of building layouts and operational functionality to help understand where there might be opportunity to increase capacity and maximise use of the building. Working collaboratively to ensure the benefit in occupation is sustained.

To represent the health centre on working groups, committees etc., as requested.

Ensure there are clear communication pathways which support and encourage all occupiers to work in a participative way, within the building operational policy.
