A new opportunity has arisen for a PCN Operations Lead to join our team as part of the Picton Primary Care Network (PCN)
NHS. This is a hugely exciting opportunity to work innovatively across our
network to support the growth of our support services within the community.
The PCN
Operations Lead will provide day-to-day operational support to the Care
Coordinator team within the Primary Care Network (PCN). Working under the
direction of the PCN Manager, they will ensure high-quality,
patient-centred delivery of PCN services, effective team operations, and
alignment with PCN priorities including practice, personalised care.
Main duties of the job
The postholder will lead and manage the Care Coordinator team, providing day-to-day supervision, performance management, and support with recruitment, induction, and training. They will be responsible for team rotas, workload allocation, and monitoring KPIs to ensure effective and high-quality service delivery. A key focus will be on fostering a positive, inclusive team culture that promotes wellbeing and continuous learning.
They will oversee the delivery of care coordination services aligned with national and local priorities, including proactive care for complex households, support for care homes, social prescribing and personalised care.
They will be required to communicate effectively and sensitively to patients, clinicians and practice admin staff across the Network GP practices.

Picton Primary Care Network Ltd consists of 6 GP practices
working together across Liverpool, with a patient list of circa 50,000
patients, to improve patient wellbeing and reduce health inequalities.

We have a well-developed, multidisciplinary team including
Clinical Pharmacists, Community Paramedics, First Contact Physiotherapists, as
well as a Mental Health Team consisting of Advanced Practitioners, Associate
Psychological Practitioners, and Mental Health Practitioners.
Our Care Coordinator team provides support across a wide
range of areas, including housebound care, cancer screening, mental health,
womens health, and chronic disease recall. They play a key role in ensuring
patients receive proactive, personalised care by helping to coordinate appointments,
follow-ups, and targeted health interventions. In addition to their day-to-day
coordination work, the team also leads on patient engagement and outreach
events within the community, helping to reduce health inequalities and improve
outcomes across the population.
