Job summary

Salford Care Centres are looking for an experienced GP Practice Nurse to be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the patient population.

The Practice Nurse will be responsible for a number of clinical areas such asinfection prevention and control,health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

You will be responsible for supporting with the supervision of our healthcare assistant and GP assistant, along with your own continued personal development and keeping up to date with nursing developments.

Main duties of the job

  • To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager
  • To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
  • To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
  • To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
  • To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
  • To diagnose and manage acute and chronic conditions
  • To co-ordinate the provision of travel medicine services
  • To request pathology services and process pathology results as required
  • To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Clinical Role

  • Manages minor illness and triage using a range of clinical assessment skills including: history-taking, physical examination, cognitive assessment and an approach that fosters a bio-pyschosocial model: critical thinking and clinical decision making: making objective and appropriate referrals

  • Formulates an action plan for the treatment of the patient with long term conditions, synthesising clinical information based on the patients presentation, history, clinical assessment and findings from relevant investigations, using appropriate evidence-based practice
  • Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff. At annual review will assess patients understanding and ability to self-manage
  • Initiate and carry out programmes of health screening as agreed with the practice clinical team: promote health and wellbeing, giving general education and advice on weight, diet and healthy eating, activity and exercise, smoking cessation, alcohol consumption and chronic disease management
  • Implements non-pharmacological related interventions/therapies dependent on situation and technical requirements of care
  • Uses new knowledge in innovative wasy and takes responsibility for developing and changing practice in complex and sometimes unpredictable environments
  • Undertake regular medication reviews for groups of patients deemed suitable by the clinical team
  • Provide up to date contraceptive and sexual health advice, including prescribing when required
  • Provide comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, sun protection, food hygiene, first aid and emergency education, health insurance and vaccinations
  • Administer child and adult immunisations and vaccinations in accordance with General Practice contract and following national and local programmes. Competent and up to date with anaphylaxis and resuscitation techniques. Up to date and follow guidelines regarding cold chain ensuring safe storage, rotation, and disposal of vaccine and drug, and that adequate stock levels are maintained
  • Competent in procedures including venepuncture, BP management, urinalysis, blood glucose testing, peak flow technique, spirometry, pregnancy testing, cervical screening, wound management, taking swabs
  • Provide support and chaperoning to GPs when required including in minor surgery clinics and other procedures
  • Enables, supports and encourages individuals, families, carers and groups to address issues which affect their health and social well being
  • Contributes to the practice achieving GMS contract obligations
  • Maintain accurate records following NMC standards of record keeping
  • Ensure all data protection requirements are met when gathering, recording and storing patient data
