PCN (SDPCN) is looking to welcome a compassionate and enthusiastic clinical practitioner into our new housebound visiting
service, Team Up South Dales.
Team Up brings joined-up, high-quality, holistic, patient-focused clinical care to patients in their own homes. The successful candidate will deliver care to patients of our 4 practices: Hannage Brook Medical Centre,
Ashbourne Medical Practice, The Surgery Ashbourne and Brailsford Medical
Practice.
The role will primarily be based at South Dales PCN in Ashbourne, Derbyshire with home visiting across the South Dales. The role is part of the wider Team Up approach to deliver joined up care to housebound people.https://joinedupcarederbyshire.co.uk/your-services/team-up-including-ageing-well/
The successful candidate will work alongside other community teams including the DCHS nursing and therapy, practice based care co-ordinators, ECPs (Community Matrons) and PCN ARRS teams.
The service hours are 8.00-6.30 Monday to Friday.
The successful candidate will report to the Team Up Clinical Lead and there is the potential opportunity to work with the PCN Lead Nurse and Lead Clinical Pharmacist on other projects.

Provide acute and proactive home visits to housebound patients registered with the South Dales PCN practices.

Essential
- Be a registered nurse or other allied healthcare professional, holding registration with the appropriate registering body in the UK with no current restrictions to practice.
- Minimum 5 years experience.
- Be able to independently assess undiagnosed and undifferentiated clinical presentations supported by a remote senior clinician.
- Be confident in a variety of settings such as remote, home visiting and clinic based.
- Embrace opportunities to work and liaise with others, across multi-professional and multi-agency boundaries, in the interests of improving patient outcomes.
- Be consistently compassionate and respectful, placing a high value on the dignity of patients, relatives, carers and colleagues.
Desirable
- Experience in primary care home visiting and/or frailty care.
- Experience in enchanced health care in care homes.
- Completed FCP roadmap.
- Familiar with TPP SystmOne.
