Job summary
We are recruiting
a practice pharmacist to fulfil a new vacancy at Ridge Green Medical Centre.
The practice is looking to recruit a competent prescriber able to
follow up patients at their annual review and look critically through repeat
medication with the patient following best practice.
Consideration
will be given to all applicants from different backgrounds and experience.

The post holder will be expected to perform medication reviews and polypharmacy reviews. They should hold an independent prescriber
qualification and be confident about drug interactions, indications and best
practices.
The post holder
will provide primary support to general practice staff with regards to
prescription and medication queries. They will help support the repeat
prescription system, deal with acute prescription requests, and medicines
reconciliation on transfer of care and systems for safer prescribing.
