Working hours:

Tuesday (Care Homes) 8.30 to 16.30

Thursday (Practice Clinic) 8.30 to 16.30

Friday (Practice Clinic) 8.15 to 12.15

Main duties of the job

The main focus for the role will be to be an integrated part of our PCN Care homes and frailty team. You will work under the supervision of our Frailty Team Leader who is a very experienced Advanced Nurse Practitioner.

Working alongside other clinical staff in the provision of patient care and under the supervision of a registered nurse.

Job responsibilities

Undertake and record the following tasks.

Urinalysis

Measurement of blood pressure, temperature, and pulse

Peak flow measurements

New patient health checks

NHS Health Checks

Hypertension monitoring and other long-term conditions

Venepuncture

Organisation of housebound annual bloods and entering health monitoring data via System one into patients records

MMSE tests

Performing ECG's

Organisation of urgent blood tests for housebound patients at short notice

Other Responsibilities

Recognise changes in patients' observations which must be reported and acted on.

Assist clinicians in emergency management of patients.

Enable patients to access appropriate professionals in the practice team.

Act as a chaperone.

Provide relevant health promotion literature to the patient.

Maintain clinical equipment efficiently.

Instruct patients in appropriate techniques for medical equipment

Interact with patients and colleagues in a professional manner

  • Skills, knowledge and experience
  • Knowledge of the Personalised care approach
  • Understanding of the determinants of health to include social, economic and environmental factors
  • Understanding of, and commitment to, equality, diversity and inclusion
  • Knowledge of IT systems to include the ability to use word processing skills, emails and internet
  • An understanding of the nature of general practice and Primary Care Networks
  • Ability to build and maintain relationships whilst maintaining appropriate professional boundaries
  • Excellent verbal communication skills with the ability to communicate effectively at all levels inc. customers, patients and carers, specialist services, GPs and colleagues.
  • Listening skills displaying empathy.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
  • Experience of using GP clinical systems, such as System One
  • Good technical literacy of Microsoft Applications e.g. Word, Excel, and Access etc.

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work)
