Job summary
An exciting opportunity has arisen for a Patient Care Coordinator within our Central Manchester PCN Hulme and City Centre South. Our PCN represents varying background and patient demographic.
The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service.
You will support The Docs, a dynamic and expanding City Centre Practice within the network to address wider health inequalities, such as cancer, mental health and vaccinations, engaging patients more widely to address the inequality needs across our patch. Your role will be to support wider health messages, help assist population health drivers and priorities plus develop welfare opportunities by speaking to patients.
Your role is an essential part of our evolving dynamic and forward-thinking PCN, working to provide enhanced care experience to patients in line with a with a view to improving health inequalities.
Main duties of the job
- Act as a point of contact between GP, patients and carers and other agencies
- Liaise with GPs and practice teams to identify patients who are elderly, frail or who have long term health needs and support
- Develop and embed the use of Group Consultations where safe to do so and help support the patients with long term conditions
- Support patients to access primary care wider services
- Raise awareness of health promotion, screening and NHS Health Checks and LD Health Checks in practices
- Assist in coordinating PCN clinics, and contact identified patients with appointments
- Manage patient initiated calls for help/signposting etc., ensuring patients are directed to appropriate service
- Document and monitor aspects of patient co-ordination and service delivery, supporting data collection and audit using the practices clinical system
- Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency situation, alerting the team or enabling a rapid response
- Support national screening and immunisation programmes and encourage uptake
- Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact
- Work closely with and refer to PCN social prescribing link workers where a patient is identified as potentially benefitting from this service
About us
The practice was established in 1992, originally running out of small rented accomodation on the Canal Street docks. In 1998 we moved to the larger Bloom Street building which has been converted into premises dedicated to health care.
With the ongoing redevelopment of Manchester city centre, the local population has changed in its demographics and continues to grow. We currently offer health care to over 10,000 residents.
Accessibility
The building is wheelchair accessible with a ground floor disabled access toilet and a lift to all other floors. We have a hearing loop at reception to assist the hearing impaired.
About the Practice
When the practice was established, our aims were simple:
- To be a fully trained and experienced team of clinicians and administrators.
- To provide a welcoming environment.
- To offer up to date medical services tailored to each individual with continuity of care.
- To cater for the needs of sections of the population experiencing a low level of care because of both the lack of NHS provision in the city centre and real or perceived discrimination, particularly on the grounds of sexuality or HIV status.
Although it is now hard to remember the days when almost no one lived in the centre of Manchester and it wasnt safe to walk between The New Union and The Rembrandt after dark, we hope that we still live up to the first three of these aims and that the fourth is no longer applicable.We are a training practice and committed to continuing medical education.
