Job summary

Main duties of the job

Nursing Associates work in practice, independently under the leadership of registered nurses, working within the sphere of nursing and care, and within all aspects of the nursing process.

About us

The practice was established in 1992, originally running out of small, rented accommodation on the Canal Street docks. In 1998 we moved to the larger Bloom Street building which has been converted into premises dedicated to health care.

We have since opened an additional site at New Jackson Medical Centre in Deansgate

With the ongoing redevelopment of Manchester city centre, the local population has changed in its demographics and continues to grow. We currently offer health care to over 12,000 residents.

Accessibility

The building is wheelchair accessible with a ground floor disabled access toilet and a lift to all other floors. We have a hearing loop at reception to assist the hearing impaired.

About the Practice

When the practice was established, our aims were simple:

  • To be a fully trained and experienced team of clinicians and administrators.
  • To provide a welcoming environment.
  • To offer up to date medical services tailored to each individual with continuity of care.
  • To cater for the needs of sections of the population experiencing a low level of care because of both the lack of NHS provision in the city centre and real or perceived discrimination, particularly on the grounds of sexuality or HIV status.

Job description

Job responsibilities

To assist and perform routine nursing tasks related to patient care, such as routine BP checks, ECG, suture and clip removal, BMI and lifestyle advice, as directed and agreed with line manager, registered general nurses and GPs.

To support patients to improve and maintain their mental, physical, behavioural health and wellbeing by using behaviour change interventions, such as undertaking health checks (physical and mental), smoking cessation services.

To support the RGN with the holistic assessment and planning of wound care plans for patients with chronic and acute wounds.

To implement agreed wound care plans, following local and national evidence-based guidelines, inclusive of the treatment of minor traumatic wounds within own level of competency. To recognise any deterioration of wounds or infection and arrange early review with PN/GP or specialist as required.

Carry out Chronic Disease Management annual reviews delegated by the Practice Nurse or senior colleague.

Support the Practice Nurse with the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach.

Assist in and perform routine tasks related to patient care as directed by the Practice Nurse GP or other member of the Primary HealthCare Team.

Be able to undertake baseline observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, spirometry, blood glucose levels, ECG, assessing nutrition and hydration, participate in screening programmes and Annual Reviews for example in patients with Learning Disabilities .

Perform cervical smear screening on completion of additional training as guidance RCN Position Statement on Registered Nursing Associates (RNAs) training in cervical sample taking.

Administer medications as prescribed and in accordance with NMC standards e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.

Following agreed protocols with referral to Practice Nurse or GPs as appropriate assist in the assessment, planning, development implementation and evaluation of programmes to promote health and well-being and prevent adverse effects on health and well-being.

Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.

Document consultations accurately and appropriately, according to NMC guideline

Ensure awareness of statuary and local clinical adult and children safeguarding protection procedures, including systems of referral. Ability to recognise signs and symptoms of abuse/neglect.

Ensure infection prevention and control guidelines are maintained.

To be responsible for providing the appropriate nursing evidenced based care of an allocated group of patients.

Be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency.

Recognise and work within the limits of their competency.

To work as part of multidisciplinary team using resources efficiently.

To deliver quality care to patients and to work effectively within the nursing team.

To be competent in carrying out a variety of clinical procedures.
