Job Summary
To
perform as an autonomous general practitioner working independently and with
other health care professionals to assess, diagnose and treat the conditions
of patients attending within a primary care setting. Provide expert professional advice to
patients, carers and colleagues and ensure the maintenance of clinical
excellence.
To
be professional accountable and responsible for all elements of clinical
practice in accordance with the GMC code of conduct ensuring the safety of
patients and quality of their care is primary.
The applicant must have been
qualified after October 2022, therefore, have been qualified for less than 2 years.
The
post hold will need to be capable of independent travel across Preston Park
Primary Care Network which includes the following practices:
Stanford
Medical Practice
Beaconsfield
Medical Practice
Preston
Park Surgery
Warmdene
Medical Practice
The
Haven Medical Practice
The
post holder will need to be flexible to meet the needs of the Business. This role is currently fixed term for 6
months but this may be subject to change.
Core Hours
Maximum
of 9 sessions a week. We are happy to
accept part time for this role, but please note this is not a remote role and
there is an expectation that the post holder will be based in practice.
Key Responsibilities
The
following are the core responsibilities of the Salaried GP.
1.
Provide patient-centred, evidence-based care
that addresses the medical needs of the local patient population. This
includes diagnosing, treating, and managing a wide variety of acute and
chronic medical conditions, working to improve overall health outcomes.
2.
Ensure that all medical services provided
align with the standards and obligations set out in the primary care
contract. This includes meeting targets related to patient care, access, and
service quality, in compliance with NHS guidelines.
3.
Implement safe, effective, and cost-efficient
prescribing practices, ensuring that medication choices follow both local
formularies and national prescribing guidelines, while considering patient
preferences and clinical appropriateness.
4.
Manage chronic conditions such as diabetes,
hypertension, COPD, and asthma through regular reviews, treatment
adjustments, and patient education, aiming to optimize control and prevent
complications. Use care plans and multidisciplinary approaches when necessary.
5.
Ensure all administrative tasks, including
managing referrals to secondary care, processing repeat prescription
requests, reviewing test results, and handling patient queries, are completed
promptly and accurately to support effective patient care.
6.
Participate in telephone triage and duty
doctor responsibilities, shared across five practices. This will involve
rotating placements, spending one month in each surgery, ensuring coverage
and continuity of care throughout the network.
7.
Ensure patient records are accurately and
promptly updated, following best practices in both EMIS and SystmOne (S1)
across all five practices to support consistent care and compliance.
8.
Contribute to the practice team by taking on a
fair and equitable share of clinical and administrative duties. This ensures
smooth operations and a balanced workload across the practice.
9.
Follow established clinical guidelines for
patient care and actively participate in audits to ensure quality
improvement. Take a proactive approach in implementing any audit
recommendations to enhance patient safety and care standards.
10.
Identify and report safeguarding concerns,
ensuring that vulnerable patients, including children and adults at risk, are
protected. Act in accordance with local safeguarding policies and participate
in safeguarding training and case discussions as required.
11.
Support practice-wide initiatives aimed at
improving patient care, service delivery, and operational efficiency.
Contribute ideas and take part in projects that drive continuous improvement
in healthcare quality and outcomes.
12.
Take ownership of specific areas of the QOF,
ensuring that the practice meets its targets. This could involve chronic
disease management, patient screening programs, or preventative health
measures, ensuring high standards of care and practice performance.
13.
Participate in practice meetings to discuss
clinical issues, operational challenges, and practice performance. Provide
input into decision-making processes and support the implementation of agreed
actions to improve service delivery.
14.
Play an active role in ensuring the practice
meets its clinical governance obligations, supporting audits, risk
management, and quality improvement activities. Help in the training and
development of colleagues, including mentoring less experienced staff.
15.
Take part in regular appraisals, reflecting on
your practice and setting goals for continued professional development.
Engage in feedback discussions and use the appraisal process to identify
areas for improvement and learning.
16.
Maintain your professional credentials and
ensure you are prepared for the GMC revalidation process. This includes
keeping accurate records of CPD (Continuing Professional Development),
audits, patient feedback, and other required documentation
17.
Take responsibility for your ongoing education
and professional development, while also encouraging a learning culture
within the practice. Keep up to date with medical advancements, attend
relevant training, and share knowledge with your colleagues.
18.
Actively participate in the education and
supervision of medical students and trainees, providing them with learning
opportunities in clinical settings, guiding their development, and ensuring
they gain the necessary skills and knowledge.
19.
Work collaboratively with practice partners to
help achieve strategic goals. Identify ways to improve service delivery,
enhance patient care, and increase practice efficiency, which could help
reduce costs or generate additional income.
20.
Ensure that you are familiar with, and
consistently follow, the practices protocols and policies in daily clinical
and administrative work. Participate in regular reviews to update protocols
as needed to meet evolving clinical guidelines and regulatory requirements.
21.
Foster a team-based approach to care by
maintaining regular communication with all practice staff. Promote a culture
of collaboration, quality improvement, and open dialogue, ensuring everyone
is working towards shared practice goals.
22.
Engage in clinical audits as requested, contributing
to the collection and analysis of data to assess and improve practice
performance and patient outcomes.
23.
Take part in initiatives that support the
improvement of local healthcare services, helping the practice to stay
engaged with wider community health strategies and to offer better care to
patients.
24.
When significant events or near misses occur, participate in reviews
and root cause analyses to identify what went wrong, learn from the event,
and implement changes to prevent recurrence.

Communication
Connect with various support agencies, both
statutory and voluntary, to ensure individuals have access to comprehensive
support. Establishing positive
working relationships and effective communication with Preston Park PCN GP
Practices, integrated care health and care teams, and a range of local
voluntary sector and community organisations.
Utilise and demonstrate sensitive
communication styles to ensure patients are fully informed and consent to
treatment.
Communicate effectively and empathetically, to overcome communication
barriers with patients and the general public displaying emotional crisis,
vulnerability, verbal/physical aggression, learning difficulties, recognising
the need for alternative methods of communication.
The ability to cope with frequently challenging, diverse and stressful
situations.
Essential
Skills/Knowledge/Experience
The applicant must not have already worked
within a GP Practice as a GP.
The applicant must have been qualified after
