Preston Park PCN are looking to recruit an enhanced practice
nurse within the ARRS roles to join our multi-disciplinary team, to help us to
transform and modernise pathways of care, enabling the safe and effective
sharing of skills across traditional professional boundaries.
The successful candidate may come from a range
of professional backgrounds. As an
experienced healthcare professional, educated to masters level, they will have
developed the skills and knowledge to allow them to take on an expanded role
and scope of practice caring for patients.
Main duties of the job
Demonstrate a high level of expertise within their
specialist service providing advice, education and support to staff, patients,
their families and carers
Be practising autonomously as an enhanced
practice nurse within the designated speciality area to provide patient-centred
clinical care. This will encompass the
skills of advanced clinical assessment, examination, diagnosis and treatment
within an agreed scope of practice throughout the directorate.
Support new ways of working that emphasises a
more efficient and patient focused service, and will ensure the safe treatment,
referral and discharge of patients with undifferentiated and undiagnosed
presentation in their area.
Have responsibility for leading the on-going
development of clinical practice and standards of care within the service,
including the development of policies, procedures, protocols and guidelines in
collaboration with multidisciplinary colleagues.

Our Primary Care Network
Preston Park PCN is a group of five surgeries - Beaconsfield Medical Practice, The Haven Practice, Preston Park Surgery, Stanford Medical Centre and Warmdene Surgery. We serve approximately 57,000 patients and aim to support and connect with our local community.
Primary Care Networks (PCNs) bringGP practices together with other local services such as community, mental health, social care and the voluntary sector to look after local populations. PCNs will help to joinupservices at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.
What are Primary Care Networks?
PCNs build oncurrent primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care.Clinicians describe this as a change from reactively providing appointments to proactively providingcare for the people and communities they serve. Where emerging PCNs are in place in parts of the country, there are clear benefits for patients and clinicians. PCNs usually have a 30-50k population and are geographically contiguous (next to ortouchinganother). Good level of communication skills both
verbal and written which can be tailored to a wide range of stakeholders
Good level of ICT knowledge and skills
Good organisational skills and time
management
Professional, calm and efficient manner
Self-motivated, can work well independently and
within a team
Works safely and effectively when working
autonomously and in conjunction with other health care professionals
