Job summary
This role involves providing strategic
and management leadership to support the PCNs delivery of a number of the
current clinical services, its QoF and IIF goals and future initiatives
aiming to improve practice resilience and the networks population health.
Working with the Networks Clinical
Director, the PCN Operational Manager, the PCN Clinical Service Teams, all five
Network practices (54k population) and other PCNs, the post holder will help
to ensure that current services run efficiently, provide good patient care
and that the PCN and its practices meet our QoF and IIF targets.
Main duties of the job
The role will also include clinical supervision and management of the Care Home / Frailty Team ANPs (and other allied clinical roles that may be employed).
The role requires the post holder to have (or wish to develop) leadership skills, good communication skills, the ability to be self-directed, and work well in a team environment

Preston Park PCN is a group of five surgeries - Beaconsfield Medical Practice, The Haven Practice, Preston Park Surgery, Stanford Medical Centre and Warmdene Surgery. We serve approximately 57,000 patients and aim to support and connect with our local community.
Primary Care Networks (PCNs) bring GP practices together with other local services such as community, mental health, social care and the voluntary sector to look after local populations. PCNs will help to join up services at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.

Exciting opportunity for 4-6 sessions per month, working with the Clinical Director and PCN management team around the planning and strategy of PCN services.
PCN Cancer Lead - developing a PCN Cancer strategy and building a team;
PCN CVD Lead - working with practices around delivering the ambition of the NHS Long Term Plan and IIF targets;
PCN QOF QI lead - working with practices to ensure meeting QoF targets;
PCN LD lead - working the PCN LD team and practice staff to improve care for LD patients;
PCN SMI Lead - working with the Clinical Director and Operational Manager with the launch of PCN MH practitioners and MH support teams and improving care and access for this cohort of patients.
Other Clinical Leads as may arise.
Regular clinical management & supervision of the Care Home / Frailty Team ANPs (and other allied clinical roles that may be employed).
Being part of Health Inequalities Team (currently looking at LD AND SMI)
Help develop clinical SOPs for other PCN projects and services, e.g. roving vaccination team and other potential projects.
Working with Practices and Practice Leads re these domains
