Job summary

The Henley Sonnet Primary Care Network is formed of The Bell and Hart Surgeries in Henley, Sonning Common Health Centre and Nettlebed Surgery. These four surgeries are working collaboratively to provide extended patient services to our community of 34,000 patients.

Henley Sonnet already employs three Clinical Pharmacists.

We plan to interview at the end of April and again at the end of May so please apply promptly. Should we successfully recruit candidates in April we may remove the job listing before the closing date.

Main duties of the job

We seek an enthusiastic and driven individual keen to join the existing pharmacist team to support primary care. This clinical pharmacy role is patient facing and includes all aspects of medicines management across both long-term conditions and acute presentations. You will work as part of highly skilled multi-disciplinary teams in each of the four practices, guiding them in innovative and cost-effective prescribing.

About us

The Pharmacy team has expanded over the last few years as the roles have developed. The team work closely with the doctors and Practice Managers in the Henley SonNet PCN. The Clinical Pharmacist team is really valued as a resource to support the surgery clinical teams.

Protected time is allocated to the Clinical Pharmacist team to work together to deliver audits, reviews, prescribing and DES requirements.

Each member of the team has a dedicated supervisor with allocated tutorial time.

Job description

Job responsibilities

Medication Reviews

Undertake clinical medication reviews with patients (including patients in care homes) and action appropriate interventions including prescribing and monitoring.

Management of medicines at discharge from hospital

To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

Clinical advice and expertise on medicines to practice staff, multidisciplinary teams and patients

Answers all medicine-related enquiries from GPs, other practice staff and patients with queries about medicines.

Signposting to community pharmacy for management of minor illness, medicines use reviews, new medicines service and other locally commissioned service. Referring to GPs or other healthcare professionals where appropriate.

Medicines Safety

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Audits

Carry out audits as required by annual Prescribing Incentive Scheme and Dispensing Services Quality Scheme.

Undertake prescribing and medicines audits as directed by GPs, feedback the results and implement changes in conjunction with the practice team.

Antibiotic Stewardship

Promotion of evidence-based practice and interventions to prevent the development of infections to practice

Responsibilities

To maintain registration as a pharmacist and comply with appropriate professional codes

As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.

To plan and organise own workload, including audit and project work, and training sessions for members of the, practice team, community nurse team, patients, and carers

To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager

To comply with all organisational and statutory requirements (e.g. health and safety, equal treatment and diversity, confidentiality and clinical governance)
