Job summary

Are you committed to making a difference in your local community? We have an exciting opportunity for one part time Community Health and Wellbeing Worker (CHWW) in Central/South Hammersmith, working in a primary care/public health role. This is a new and exciting role - the outreach work of a CHWW is essential in addressing the rising inequities in health and social care and in identifying unmet need. Your aim is to contribute to improved health and wellbeing through a range of activities such as outreach assessments, community education, signposting, informal counselling, and advocacy. This is an exciting opportunity to shape a relatively new job role in the UK, supported by peers working in other personalised care roles. The role will be hosted by one of the three Primary Care Networks in the H&F Collaborative: South Fulham, Hammersmith & Fulham Partnership or GP at Hand.

Main duties of the job

Monthly household visits (or more frequent if the household need requires it) within a defined geographical area (up to a maximum of 120 households within defined areas) to assess the health and social needs of everyone within a household, adopting a proactive and holistic approach when supporting the local community.

  • Completion of a higher education certificate or diploma which has included public contact through internship, volunteer or other experience
