Job summary

We are seeking an experienced and motivated Respiratory Nurse to join our Primary Care Network (PCN) on a part-time basis.

The post holder will play a key role in delivering high-quality respiratory services across our member GP practices, with a particular focus on providing and supporting spirometry testing in line with national standards.

This is an excellent opportunity to work collaboratively within a multidisciplinary team to improve respiratory outcomes for patients across the PCN.

Main duties of the job

  • Provide high-quality, accurate spirometry testing across all practices within the PCN
  • Ensure spirometry is conducted in line with ARTP guidelines and accreditation standards
  • Support the diagnosis and ongoing management of patients with respiratory conditions, including COPD and asthma
  • Interpret spirometry results and communicate findings effectively to clinicians and patients
  • Work collaboratively with GPs, practice nurses, and other members of the multidisciplinary team
  • Contribute to the development and improvement of respiratory services within the PCN
  • Maintain accurate clinical records in line with organisational policies
  • Support training and education of staff where appropriate

About us

GP Connect PCN consists of 5 GP Practices covering approx. 32000 patients. Our network operates with in the city of Coventry.

The successful candidate will join a GP Connect PCN Team and will be supported by a Senior Team and Regular development sessions, teaching, training and opportunities to upskill will be provided.

Key Responsibilities

Clinical Practice and Care Delivery

  • Deliver respiratory clinics for patients with asthma, COPD and other respiratory conditions within agreed scope of practice.
  • Undertake holistic assessment, care planning, treatment optimisation and review for patients with chronic respiratory disease.
  • Provide follow-up support for patients after exacerbations or hospital discharge, including home visits where clinically appropriate.
  • Contribute to respiratory diagnostic activity, including spirometry and FeNO testing, where competent and trained.
  • Identify patients who may benefit from pulmonary rehabilitation, smoking cessation support, home oxygen assessment or other specialist input, and refer appropriately.
  • Support delivery of virtual reviews, group consultations and shared clinic models where required.

Integrated Working and MDT Contribution

  • Work collaboratively with GP practices, the Respiratory Nurse Specialist, pharmacists, healthcare assistants, social prescribers, community teams and secondary care services.
  • Attend and contribute to respiratory multidisciplinary team meetings and other relevant case discussion forums.
  • Support seamless pathways between primary care, community services and secondary care to improve patient outcomes and experience.
  • Liaise with carers, families and other professionals to coordinate patient-centred care plans.

Education and Support

  • Contribute to service development, audit, quality improvement and pathway standardisation across the respiratory service.
  • Provide education and self-management support to patients and carers to improve confidence, understanding and adherence to treatment.
  • Support practice staff with respiratory knowledge and skills development, including inhaler technique, annual review processes and diagnostic pathways, within own competence.
  • Participate in education sessions for healthcare professionals and the local population as required.
  • Maintain own continuing professional development and work towards agreed respiratory competencies and qualifications.

Service Improvement, Audit and Quality

  • Contribute to service development, audit, quality improvement and pathway standardisation across the respiratory service.
  • Support data collection, accurate coding, clinical documentation and outcome monitoring using agreed systems such as EMIS WEB.
  • Participate in reviewing clinical activity and identifying opportunities to improve access, quality and consistency of respiratory care.
  • Work in line with local and national guidelines, PCN policies and evidence-based practice.

Professional and Governance Responsibilities

  • Maintain professional registration and practise in accordance with the NMC Code and all relevant clinical governance requirements.
  • Work within own scope of practice, recognising when to seek support, escalate concerns or refer to senior clinicians.
  • Participate in supervision, appraisal, mandatory training and clinical competency development.
  • Maintain confidentiality, safeguard vulnerable people and comply with information governance, infection prevention and health and safety requirements.
