Job summary
An exciting opportunity has become available for a Registered Nurse, to join our experienced and enthusiastic team of nurses within a large, busy GP practice to train in the management of chronic disease particularly diabetes care. We are looking for someone who is motivated and enjoys working as part a team. We have a vibrant multicultural practice population and work closely with the wider multidisciplinary teams.
This post is suitable for a newly qualified nurse looking to work in general practice or a nurse who would like to train in chronic disease management (or already has existing experience in diabetes care). You will work alongside Advanced Nurse Practitioners with many years experience working in these fields.

Main duties of the job
- Chronic disease management - primarily diabetes care (to be trained in this area or use existing skills/knowledge
- Telephone triage and minor illness is desirable but not essential.
