Job summary

We have an exciting opportunity for 2 Advanced Nurse Practitioners to join our multi-professional Primary Care Team working within our new Health & Wellbeing Hub under the direct supervision of the Clinical Director.

The purpose of this role will be to fulfil the requirements of an experienced practitioner acting within their professional boundaries and will be professionally registered. They will provide care to the presenting patient and be expected to work autonomously but within the PCNs protocols and procedures as directed by the Clinical Director. They will also be able to demonstrate safe clinical decision making including high levels of critical thinking and holistic practice.

Main duties of the job

The post holder will be required to take an active role to support efficient and effective service delivery to the Primary Care Network (PCN) population.

To take an active role in supporting the PCN to maximise achievement against local and national targets

Maintain accurate and contemporaneous records within the protocols and systems used by the PCN.

To attend any relevant PCN meetings, educational events and training forums as required.

To support and manage other clinical PCN team members working from within the Health and Wellbeing hub to include Nurse Associates, Health Care Assistants and Phlebotomists.

The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the PCN.

The postholder will have a good understanding and practical experience of working with patients with long term conditions and understand spirometry and feno diagnostic systems or a willingness to undertake training.

The post holder will work collaboratively with the multi-disciplinary PCN team to meet the needs of patients, supporting the delivery of policy and procedures.

The post holder will work within the clinical on-the-day team to deliver excellent primary health care services.

As a PCN we are forward thinking, innovate and driven to deliver the best patient care for our population. This includes health population management, and this role ties in with supporting that and tracking the improvements we can make to patients lives.

We have recently started an exciting joint venture with Somerset NHS Foundation Trust to open a Health and Wellbeing Hub at the old Victoria Park Medical Centre. This is a flagship hub, the first of its kind that will bring together Primary and Secondary care all under one roof to support the Bridgwater Bay community.

Accurately examine and assess patients with an undifferentiated diagnosis, from a physiological, psychological and social perspective, and plan clinical care accordingly.

Manage and support patients with chronic long term conditions working with the wider services and healthcare professionals to access other care pathways as required.
