Job summary

We have an exciting opportunity for 2 Advanced Nurse Practitioners to join our multi-professional Primary Care Team working within our new Health & Wellbeing Hub under the direct supervision of the Clinical Director.

The purpose of this role will be to fulfil the requirements of an experienced practitioner acting within their professional boundaries and will be professionally registered. They will provide care to the presenting patient and be expected to work autonomously but within the PCNs protocols and procedures as directed by the Clinical Director. They will also be able to demonstrate safe clinical decision making including high levels of critical thinking and holistic practice.

Main duties of the job

The post holder will be required to take an active role to support efficient and effective service delivery to the Primary Care Network (PCN) population.

To take an active role in supporting the PCN to maximise achievement against local and national targets

Maintain accurate and contemporaneous records within the protocols and systems used by the PCN.

To attend any relevant PCN meetings, educational events and training forums as required.

To support and manage other clinical PCN team members working from within the Health and Wellbeing hub to include Nurse Associates, Health Care Assistants and Phlebotomists.

The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the PCN.

The postholder will have a good understanding and practical experience of working with patients with long term conditions and understand spirometry and feno diagnostic systems or a willingness to undertake training.

The post holder will work collaboratively with the multi-disciplinary PCN team to meet the needs of patients, supporting the delivery of policy and procedures.

The post holder will work within the clinical on-the-day team to deliver excellent primary health care services.

As a PCN we are forward thinking, innovate and driven to deliver the best patient care for our population. This includes health population management, and this role ties in with supporting that and tracking the improvements we can make to patients lives.

We have recently started an exciting joint venture with Somerset NHS Foundation Trust to open a Health and Wellbeing Hub at the old Victoria Park Medical Centre. This is a flagship hub, the first of its kind that will bring together Primary and Secondary care all under one roof to support the Bridgwater Bay community.

Job responsibilities

Management of patient health and illness

Accurately examine and assess patients with an undifferentiated diagnosis, from a physiological, psychological and social perspective, and plan clinical care accordingly.

Manage and support patients with chronic long term conditions working with the wider services and healthcare professionals to access other care pathways as required.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients in care and nursing homes and visit patients in their homes as required.

Provide information and advice on prescribed or non-prescription medication, on medication regimens, side-effects and interactions.

Ensure full and accurate documentation in patient notes of clinical history, physical examination, diagnosis and treatment plan.

Where the post holder is an independent prescriber, to prescribe safe, effective and appropriate medication as defined by current legislative framework.

Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.

Pro-actively promote the role of the ANP within the PCN, and externally to key stakeholders and agencies.

To play an active role in the development of PCN development plans and new services.

Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Team role

To liaise and work with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector and CCG in order to assure appropriate care is provided for the PCN population.

To participate as a key member of the multi-professional team through the development of collaborative and innovative practice.

To value all team members and encourage an environment that supports peer review / peer learning.

To encourage self-assessment and challenge any complacency or actions that are not in the interest of the public and/or users of services.

To anticipate barriers to communication and take action to improve communication.

To support the delivery of health and safety guidance as agreed within the PCN.

To support the writing of maintenance and delivery of CQC guidance within the PCN.

Organisational

To complete records, audits, reports and respond to appropriate questions and requests.

To attend meetings as requested.

To keep the Primary Care Network Manager informed about pressures and difficulties or problem areas when aware.

To contribute to the PCN strategy and developments when requested, including the contribution to the development of PCN protocols, policies and standards.

To work according to Clinical Governance and support the Clinical Governance Agenda.

To contribute to risk assessments, audits and Significant Events and keep informed of the importance of Infection Control.
