Job summary

We have a great opportunity for 2 Nursing Associatesto join our multi-professional Primary Care Team working from our new Health & Wellbeing Hub under the direct supervision of the Clinical Director.

This role is part of the extended hours provision and will include Saturdays on a rota basis.

Main duties of the job

Clinical

  • Undertake base line observations including BP, pulse, respirations, weight, height and glucose monitoring.
  • Requesting pathology tests, for example urine culture, swabs.
  • Venepuncture, ECGs and routine observations.
  • Wound care and removal of sutures/clips.
  • Annual reviews and management of chronic diseases including asthma, COPD, hypertension and diabetes (additional, accredited training will be provided for this).
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Home visits including Care and Nursing Homes
  • Effectively manage own time, workload and resources.
  • Report any safeguarding issues concerning children or vulnerable adults.
  • Work effectively with individuals in other agencies to meet patients needs.
  • Any other delegated duties appropriate to the post

Communication

  • Maintain effective communication with staff members, patients, carers and other members of the primary care team.
  • Refer patients directly to other services/agencies as appropriate, to promote integrated and seamless care.
  • Maintain accurate and complete patient records of patient consultations and enter on to computer systems using agreed templates and read codes.
  • Communicate clearly and appropriately, adopting the principles of safe practice and assessment of competence.
  • Attend and participate in practice and clinical meetings.

Bridgwater Primary Care Network (PCN) is the largest PCN in Somerset with 9 GP practices and a diverse population spread across town and rural locations.

As a PCN we are forward thinking, innovate and driven to deliver the best patient care for our population. This includes health population management, and this role ties in with supporting that and tracking the improvements we can make to patients lives.

We have recently started an exciting joint venture with Somerset NHS Foundation Trust to open a Health and Wellbeing Hub at the old Victoria Park Medical Centre. This is a flagship hub, the first of its kind that will bring together Primary and Secondary care all under one roof to support the Bridgwater Bay community.

Essential

  • Registered Nursing Associate with NMC PIN
  • Nursing Associate foundation degree qualification
  • GCSE Grade A-C in Mathematics and English or skills level 2 Mathematics and English or equivalent

Experience

Essential

  • Experience of working as a Nursing Associate
  • Worked within a healthcare/medical environment
  • Experience of phlebotomy, ECGs, wound care, blood pressure and health checks
  • Working within a patient facing environment
  • Ability to support patients and provide high standards of care

  • Ability to recognise and work within limits of competence and seek advice when needed
