Job summary

Shrewsbury Primary Care Network (PCN) is seeking a highly motivated and experienced Advanced Practitioner to join our dynamic and multidisciplinary team. This is a pivotal role designed to enhance the delivery of high-quality, patient-centred care across our network.

The post-holder will be a non-medical prescriber, who is motivated, proactive, committed and has excellent communication skills.

Main duties of the job

The successful candidate will be working in our Winter Illness Centre, which is based at the Shrewsbury Health Hub (Severn Fields Health Village, SY1 4RQ), managing acute respiratory conditions during the winter period. They will be expected to see, assess and manage their own list of patients, with appropriate support and supervision.

Going forward the post holder will be supporting the PCN in the delivery of the PCN Directed Enhanced Service (DES) specification by working closely with the PCN Senior Leadership Team. The Advanced Practitioner will have a clinical interest in respiratory and/or frailty.

Job responsibilities

Winter Illness Centre

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological perspective, and plan care accordingly.
  • Work collaboratively as part of a multidisciplinary team, supporting GPs and other colleagues within the Winter Illness Centre.
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, utilising national and local guidelines.
  • Work with patients to support compliance with and adherence to prescribed treatments.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
  • Maintain accurate, timely clinical records in accordance with local and national standards.

Deliver of the PCN Directed Enhanced Service (dependent on skills and interests)

  • Contribute towards the delivery of multidisciplinary proactive care for complex patients at risk of deterioration through frailty.
  • Seek to improve health outcomes for the PCN population by providing specialised care and education to individuals with asthma, including assessing asthma control, teaching inhaler technique, developing personalised asthma plans and providing self-management strategies.
  • Support ongoing service development and quality improvement within the PCN.
  • Contribute to teaching, training and mentoring of colleagues and students if appropriate.
  • Critically appraise and synthesize information from a variety of sources such as outcomes of relevant research, quality improvement projects, evaluations and audits.
  • Role model high levels of professionalism to people in the organisation and wider networks.
  • Demonstrate tactical leadership by working collaboratively with a broad range of people within and across the organisation and with external stakeholders and agencies.
  • Provide support, leadership and mentorship to any member of the team as required.

  • Minimum of 3 years' experience
  • Experience in the acute examination of adults and children
  • Management of common minor injuries/illnesses
  • Experience as an autonomous practitioner
  • Able to work under pressure and manage varying workloads and changing demands
  • Evidence of ongoing post registration CPD
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Experience of working within a multidisciplinary team
  • Experience with clinical risk management
