Job summary
A Nurse Practitioner at Shires Health Care will be an
experienced nurse who, within their professional boundaries, will provide
autonomous care for patients at the practice from initial history taking,
clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe clinical decision
making and a high level of independent care including assessment, diagnostic
and prescribing skills. This will include
the treatment of both urgent patients and patients requiring long term
management of chronic conditions. They
will work as an integral part of Shires Health Care GP and Nursing team,
supporting the delivery of the practice policies and procedures incorporating
clinical leadership skills.
Main duties of the job
To recognise and work within own competence
and professional code of conduct as regulated by the NMC.
Assess and treat patients presenting with
undifferentiated and undiagnosed acute illness or long-term conditions; to
include adults, children, mental and physical health.
Undertake selected home visits for acute
illness and long-term condition management if needed.
Undertake telephone triage and telephone
consultations and answer queries.
Clinically assess, diagnose, plan,
implement and evaluate interventions and treatments for patients with
potentially complex needs.
To order diagnostic tests and initiate
referrals as appropriate. To admit
directly to hospital in acute medical circumstances and undertake a
comprehensive handover.
To interpret
findings/reports and take responsibility for following up patients. All referrals should be appropriate and cost
effective and, as such, a NP would be expected to participate in peer review.
To provide safe, evidence based, cost effective,
individualised patient care adhering to practice policies and procedures and
promote consistency of care.
To prescribe (adhering to national, Area
prescribing Team and Practice Formulary unless clear justification) and
integrate both drug and non-drug-based treatments into a management plan. Provide information and advice to the
patient/carer on prescribed or OTC medication on regimens, side effects and
interactions.
