Job summary

The role will involve playing a full and active role as part of the practice multidisciplinary team.

The ideal candidate for the role will have the following:

  • Previous GP Practice experience
  • At least three years of post-graduate experience
  • Excellent Clinical knowledge.
  • Previous experience working with Emis and Docman
  • The commitment to continue to learn and develop.
  • The confidence to work as part of a busy GP practice
  • The ability to adapt to convey information to patients and fellow healthcare professionals.

Main duties of the job

The post holder will help patients to get the best from their medicines by improving repeat prescribing processes in General Practice, and aiming to reduce wasted medicines.

Responsibilities and Duties

  • Respond to medication queries/shortages/recalls
  • Lead on medication synchronisation to support patients and community pharmacy to mange patient medication ordering
  • Complete medication reviews
  • Medication audits.
  • Working with the Multi-Disciplinary Team
  • Processing clinical letters
  • Identify opportunities for medication cost savings.
  • Act as a key contact in practice for all medication-related queries.

Job responsibilities

Clinical Pharmacy in General Practice, our goals are to help keep patients safe and well out of hospital and reduce the demands on general practice regards medicine use. The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations processes, while addressing both the public health and social care needs of patients in the GP practice. The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice. As well, the post holder will develop and encourage a positive culture of medicines optimisation. General requirements The post holder is expected to Support a workplace culture that values equality and diversity principles towards other staff and patients. Ensure principles of openness, transparency and candor are observed and upheld in all working practices including respect for information and clinical governance, and safeguarding responsibilities.

Personal & Professional Development The post-holder will take responsibility for their own development, learning and performance by actively participating in any external training GP practices Promote high quality cost-effective and evidence-based use of medicines within the PCN, Work within the Pharmacy Team, with other clinical pharmacist and Pharmacy Technician, to respond to medication-related queries generated by patients, GPs, other health care professionals and staff. Undertake clinical medication reviews either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities. Identify and produce recommendations for medication optimisation for GPs and other multi-disciplinary team members. Contribute to the safety of drug monitoring processes and drug alerts. Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
