Job summary
An exciting opportunity has
arisen for a Patient Care Coordinator within Five Oaks Family Practice, a
well-established GMS Practice based in Beswick, Manchester.
This new and evolving role in General Practice
is being developed to improve the quality of patient care and operational
efficiencies within the practice and will require motivation and determination
to deliver excellent service.
Main duties of the job
Your role will be to support
wider health messages and to help assist with improvement in the health and
wellbeing of patients and their carers.Working
closely with the Practice clinicians and thewider MDT team,the PCC
co-ordinates patients healthcare and directs them to the appropriate service
to ensure that they get the most suitable care from whatever health or social
care provider is appropriate.
