Job summary

We are looking for a 30 hour per week, Clinical Pharmacist to join our clinical team at Neasham Road Surgery. This role is funded with ARRS but is permanent. We are a busy General Practice with a list size of 13,250. We are looking for a hard working applicant to join a team of 5 GP's, 4 ANP's, Pharmacy Technician, host of Practice Nurses and admin staff to provide a robust and effective prescribing and medication service to our patients. We can provide you with a supportive role along with some humour on a day to day basis. Ideally you will have primary care experience.

Main duties of the job

We are looking for a flexible candidate who can work alongside our current Pharmacy Technician as well as the GP's and ANP's in the day to day role of prescribing, repeat authorisations, medication reviews (face to face or telephone) and CQC medication reports along with other ad hoc pieces of work involving prescribing. In addition, you will play a supportive role to the Pharmacy Technician and administrative prescribing team. Attendance at PCN and other network meetings is required. We can offer a supportive and welcoming place of work, GP supervision if required, inclusion and attendance at Practice meetings. We are a busy practice (2nd largest in Darlington) but thrive on the this and look forward to the next social outing.

About us

Neasham Road Surgery is a purpose built surgery with access to the main train station and bus routes. We have a pharmacy opposite and close networks with our neighbouring practices and PCN. The practice has close links to the GP federation in addition. We currently have planning permission to extend the building which will in time, aid us to expand our clinical team.

The chosen candidate will have a supervising GP with regular supervision meetings, although we do require the candidate to be able to use their knowledge and experience on a day to day basis. We encourage further training for all of our staff.

We are a friendly bunch, who support each other on a daily basis whilst ensuring our patients receive a high quality service on a day to day basis.

Job responsibilities

The Clinical Pharmacist will support the Partners and other clinical staff including the Pharmacy Technician in the surgery to deliver a programme of medicines optimisation, including managing hospital discharge medication changes, secondary care implemented medicines, CQC related medicine changes and practice medication queries. The post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation for the clinical staff within the Practice.

Key responsibilities

Be an independent prescriber

Contribute to the delivery of effective daily processes for the transfer of care between hospitals and clinical staff including effective signposting, recommendation of medication changes and communication accordingly

Monitoring creatinine clearance for DOACs and checking correct doses which will include changing appropriate patients to edoxaban

Support colleagues and patients with medication queries and monitoring

Support clinical colleagues with the daily repeat authorising tasks and prescribing requests

Undertake clinical medication reviews to proactively manage people with complex polypharmacy

Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients

Using audit and action planning, help to tackle inequalities

Provide leadership on person-centered medicines optimisation (including ensuring prescribers in the Practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement

Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services

Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets)

Work with the General Practice team to ensure that the practice is compliant with CQC standards where medicines are involved

Take a lead role in some aspects of supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload

Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system

Take a central role in the clinical aspects of shared care protocols, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulation services

Undertake the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education Pathway or equivalent primary care training pathway if not already underway or completed

Provide Clinical Supervision for the Practice Pharmacy Technician
