Job summary

A new and exciting opportunity has arisen for a Physician Associate to join our practice.

We are looking for a highly motivated, reliable, and friendly professional to join our small, well-established team for a new full-time role.

We are a 6000+ patients training practice, based on one single site in a fast developing area of South East London, with Southwark Park at our doorstep, 3 minute walk from Surrey Quays Overground Station, 11 minute walk from Canada Water Jubilee Line station.

We use Emisweb, Docman, T-Quest, Accurx and DXS referral forms. We are always keen to explore and improve in order to offer the best possible care, such as organising mindfulness breathing workshops for patients and staff. We are also part of the the RCGP national virology survey.

Main duties of the job

This post is principally aimed to further expand our care for patients living with chronic conditions. The successful candidate will be working in conjunction with our ANP and two GP partners delivering proactive and empowering care to our community of patients with chronic diseases. The post will entail performing holistic assessments and regular chronic condition reviews, promoting lifestyle changes and assessing adherence to treatments. This will be delivered mainly via face to face appointments- either at the surgery or at patients homes, but there will be scope for telephone consultations also.

You will be supervised by and working side by side with one of the clinicians. Whilst we will ensure that you are competent for the initial requirements of your post, we shall also aim to foster improvement on your clinical skills and growth in confidence and expand on your role. You will be mentored by an accountable clinician who will encourage and support you to develop a professional development plan as well as explore training opportunities.

You will follow recommended annual appraisals and continuing professional development set out by the Faculty of Physician Associates (FPA).

About us

You will be working as a valuable member of our multidisciplinary team which includes a health care assistant, a nurse, a mental health practitioner, a first contact physiotherapist, a pharmacist , an Advanced Nurse Practitioner and five GPs, as well as our lovely reception administration team and practice manager.
