Job summary

We have an exciting opportunity for a motivated and enthusiastic Pharmacy Technician to join our Primary Care Network, supporting our 4 GP practices across the Wimborne & Ferndown area.

The successful candidate will work as part of our Network Pharmacy Team which consists of 3 Clinical Pharmacists and 2 Pharmacy Technicians who are supported by the GP Clinical Lead. Our new Technician will work with the team to optimise the systems and processes in place and to resolve prescription related issues.

The clinical pharmacy in general practice model is supported by the direction of national policy where there is a need to better utilise the role of pharmacy professionals within primary care to pro-actively help patients stay safe as well as helping to reduce the demands on general practice.

Main duties of the job

To help achieve efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices.

To promote high quality cost-effective and evidence-based use of medicines within the PCN.

To work within the Pharmacy Team, with clinical pharmacists to respond to medication-related queries generated by patients, GPs, other health care professionals and staff .

To work within the Pharmacy Team, with clinical pharmacists to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services

To process medication changes from tasks and hospital discharge letters.

About us

Wimborne & Ferndown PCN provides services to 42,000 patients across 4 Practices (Quarter Jack Surgery/Old Dispensary, Walford Mill Medical Centre, Pennys Hill Practice, Orchid House Surgery). We have adopted new roles within the PCN to support and broaden our primary care team which allows us to provide better care for our patients.

The PCN is headed by our committed Clinical Directors and PCN Business Manager.

In addition to the PCN, our Practices are also part of Castleman Healthcare, a non-profit company owned by 13 GP Practices in North Poole and East Dorset (3 PCNs). Castleman Healthcare offer additional services to our Patients through separate contracting arrangements, provide business and practical support to the PCNs and support with representing the PCNs voice within the wider healthcare system.

Job description

Job responsibilities

Clinical

  • Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
  • Review Hospital Discharge Letters and impliment medication adjustments as required.
  • Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
  • Support the Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR
  • Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
  • Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

Technical and Administrative

  • Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.
  • Implement efficient ordering and return processes and reducing medication wastage.
  • Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).
  • Promotion of Electronic Repeat Dispensing (eRD) and online ordering
  • Develop relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
  • Support the PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services
  • Supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services;
  • Support the PCN in reviewing and developing practice policies for CQC requirements
