An exciting opportunity has arisen for an ANP to join Dartford MODEL Primary Care Network as part of their growing multi-disciplinary team. We pride ourselves on providing excellent patient centred care and wish to expand our team to deliver high quality services across our network.
The post holder will, whilst working within their professional boundaries, provide care for the presenting patient from initial history taking,clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. This will include both acute and chronic disease management.
Main duties of the job
- Providing the both telephone and face to face consultations
- Making professionally autonomous decisions.
- Providing a first point of contact for patients.
- Instigating necessary invasive & non-invasive diagnostic tests or investigations & interpret findings/reports.
- Prescribing safe, effective & appropriate medication as defined by current legislative framework.
- Providing safe, evidence-based, cost-effective, individualised patient care
- Offering a holistic service to patients & their families.
- Promoting health & well-being through the use of health promotion, health education, screening & therapeutic communication skills.
- Referring patients directly to other services/agencies.
- Working as part of the practice team in developing & delivering specialist primary care services.
- Screening patients for disease risk factors & early signs of illness.
- Working with multi-disciplinary team to promote integrated & seamless pathways of care.
- Contributing to the practice achieving its quality targets.
- Participating in the identification of community health needs & develop strategies to address them.

About us
Dartford MODEL PCN is based in Dartford, Kent. The PCN consists of three practices; The Wellcome Practice, Dr Shimmins and partners (Dartford East) and Lowfield Medical Centre and covers around 33,000 patients.
We have a dedicated team of ARRS roles including, paramedics, pharmacists, a pharmacy technician, physiotherapists, a mental health practitioner, GP assistants and care coordinators, working alongside the practices to support patient care. We are now looking to expand the team further with the addition of an advanced nurse practitioner.
