Job summary

This role is primarily to work within our care homes.

The majority of the role is to work as an advanced nurse practitioner providing high quality and timely patient centred care within the care homes allocated to supporting the delivery of the Enhancing Health in Care Homes Network DES specification. The post holder will be responsible for the assessment, diagnosis, treatment and referral of care home patients within their level of competence and agreed protocols. This role will lead the wider care home team which includes: clinical pharmacists and pharmacy technicians, a paramedic, a healthcare assistant and administrative support. The team collaboratively deliver both acute and urgent care, long term condition reviews, personalised care planning and general support to the care homes.

Main duties of the job

Assess, diagnose, plan, implement & evaluate treatment/interventions & care for pts with an undifferentiated diagnosis

Assess, diagnosis, plan, implement & evaluate interventions/treatments for pts with complex needs. Identify, diagnose & manage treatment plans for pts at risk of developing a longterm condition

Diagnose & manage acute & chronic conditions, integrating both drug & non drug based treatment methods management plans

Prescribe & rv medication for therapeutic effectiveness, appropriate to pt needs & in accordance with evidence-based practice & national & practice protocols

Work with pts to support compliance with prescribed treatments. Provide info & advice on prescribed or over the counter medication on medication regimens, side-effects & interactions

Identify & refer pts presenting with mental health needs in accordance with the NSF for Mental Health Implement & participate in vacs & imms programmes for adults & children

Wound care

Assessment upon admission into carehomes

Weekly support to the homes

Advanced end of life care planning to include ReSPECT discussions & development of Personalised Care & Support Plans

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
