Job summary

We are looking for an Advance Clinical Practitioner / Paramedic Practitioner / Nurse Practitioner to work in our Urgent Treatment Centre.

The post holder is an experienced practitioner, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe clinical decision making and expert care for patients within the Urgent Treatment Centre. They will work collaboratively with the multi-disciplinary team and meet the needs of patients, supporting the delivery of policies and procedures, and providing leadership as required.

In this role all employees will be expected to behave at all times in a way that is consistent with and actively supports our organisational values.

Main duties of the job

Scope and Purpose of the Role:

To deliver a high standard of patient care as Practitioner in urgent, unscheduled care using advanced autonomous clinical skills, and a broad and in depth theoretical knowledge base.

To manage a clinical caseload, dealing with presenting patients needs.

To lead the team on duty (where applicable) and ensure the highest standards of care are provided to patients.

Job responsibilities

Clinical Role-

Make professionally autonomous decisions for which the practitioner is accountable.

Provide the opportunity of direct access to patients who present at any time between 8am- 8pm.

Make use of skills in history taking, physical examination, problem solving and clinical decision making to establish a diagnosis and management plan.

Instigate necessary diagnostic tests and investigations with informed consent from the patient and interpret findings/reports. Eg x-ray.

Provide safe, evidence based, cost effective, individualised patient care.

Offer a holistic service to patients and their families developing, where appropriate, an on-going plan of care/support, with an emphasis on self care.

Promote health and well-being through the use of health promotion, health education and therapeutic communication skills.

Refer patients directly to other services/agencies as appropriate.

Contribute to the unit achieving high standards of patient care and service delivery.

Teaching and Mentoring Role:

Promote a learning environment for patients and healthcare professionals.

Contribute to the implementation of the teaching for existing staff and students.

Professional Role:

Promote evidenced-based practice through the use of the latest research-based guidelines and the development of practice-based research.

Monitor the effectiveness of your own clinical practice through quality assurance strategies such as the use of audit and peer review.

Maintain professional registration.

Participate in continuing professional development opportunities to ensure that up to date evidence based practice knowledge and competence in all aspects of the role is maintained.

Work within the NMC Code of Professional Conduct or the HPCP Code of Professional Conduct.

Record accurate consultation data in patients records in accordance with the most up to date NMC guidance and other pertinent standards.

Keep up to date with relevant health related policy and work with the practice team to consider impact and strategies for implementation.

Essential

  • Post registration experience with at least one year in primary care, A&E, UTC or alternative unscheduled care context.
  • Experience working to protocols, guidelines and patient group directives (PGDs).

Desirable

  • Experience of working with a range of age groups including children.

Knowledge

  • Ability to work autonomously.
  • Knowledge of unscheduled care practice.
  • Excellent Clinical Skills.

Qualifications
