Job summary

We are looking for an enthusiastic, experienced Practice Nurse to join our friendly and supportive GP practice.

You will be vital in delivering high-quality care to our patient population of approximately 9000. Duties will including chronic disease management, immunisations and screening services.

Main duties of the job

As a Practice Nurse you will deliver high quality patient care and organisational qualities which will include:

Management of chronic disease clinics

Childhood immunisation programmes and all other aspects of immunisations

National screening procedures

You will also be expected to work collaboratively with the wider Practice team and Primary Care Network.

The practice is high QOF scoring and CQC rated Good, with excellent long-term staff retention. We are an active member of the Sale Central PCN.

We have 4 GP partners, 5 salaried GPs, 2 Practice Nurses, a HCA and a large administration team.

We are a training practice and host ST1, ST2, FY doctors. Many of which have returned as a permanent member of the team.

  • Experience of working in a primary care environment as a Practice Nurse
  • Experience of working under pressure
  • Clinical knowledge in the following:
  • ECGs
  • Venepuncture
  • Immunisations (routine, childhood and travel)
  • Womens health (cervical cytology, contraception, etc.)
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Travel medicine
  • Chronic disease management awareness including: Diabetes, Hypertension, Asthma, COPD, CHD, CKD, Stroke
  • Be willing to undertake home visits
  • Understanding the importance of evidence-based practice
  • Understand the requirement for PGDs and associated policy
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understanding when to refer to GPs
