Job summary
Winchester City Primary Care Network are seeking an enthusiastic and experienced Band 7 Pharmacists to play a key role in the successful delivery of the Pharmacy Service.
Due to the rapid and positive development of this service we plan to expand and add to our well established team of Clinical Pharmacists and Independent Prescriber.
These roles will suit candidates who are considering a split of their working hours across difference sectors, experienced Pharmacists with an independent prescribing qualification as well as less experienced Pharmacists as full CPPE funded training will be provided.
Main duties of the job
The Pharmacists will work within their clinical competencies as part of a multi-disciplinary and multi-site team, providing expertise in clinical medicines management.
Providing structured medication reviews of patients with polypharmacy, especially for older people, people in residential care homes and those with multiple comorbidities. They will support the management of long term conditions and the management of medicines on transfer of care, as well as systems for safer prescribing. They will also manage repeat prescription authorisations, reauthorisation and aute prescription requests, while addressing both the public health and social care needs of patients the PCN.
The post holders will take part in quality improvement initiatives and clinical audit, support of delivery of key IIF Indicators as well as supporting the Quality and Outcomes Framework (QOF).
Applicants will need excellent communication and interpersonal skills and have experience working as an autonomous clinical pharmacist, preferably in a similar role.
The roles require accreditation in the 18 month CPPE Primary Care Pharmacy Education Pathway; Pharmacists who have already completed a previous CPPE pathway, or have a considerable amount of prior knowledge, skill and experience that they can demonstrate, may be exempt from part/all of the pathway
