Job summary

Winchester City Primary Care Network are seeking an enthusiastic and experienced Pharmacist Service Manager to play a key role in the successful delivery of the Pharmacy Service.

Due to the rapid and positive development of the pharmacy service we plan to expand and add to our well established team of Independent Prescribing Pharmacists, Clinical Pharmacists and Pharmacy Technicians by recruiting a Clinical Pharmacist Service Manager.

The Clinical Pharmacist Service Manager for Winchester City Primary Care Network (PCN), will work as part of the multi-disciplinary general practice team in a role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

The post holder will be a prescriber, or will be completing training to become a prescriber, and will work with and alongside the general practice team.

The post holder will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD , asthma, depression) and people with learning disabilities or autism to implement the structured medication review and optimisation set out in the Primary Care Network Contract Direct Enhanced Service (DES).

Main duties of the job

The Clinical Pharmacist Service Manager will provide specialist expertise in medicines, addressing the public health and social care needs of patients across PCN practices while tackling health inequalities. They will lead person-centred medicines optimisation, ensuring prescribers follow local antimicrobial stewardship, improving quality, and contributing to the Quality and Outcomes Framework (QOF) and enhanced services. Through structured medication reviews (SMRs), they will help patients optimize medication use, reduce waste, and promote self-care.

They will also lead efforts to integrate general practice with wider healthcare teams, including community and hospital pharmacies, to improve patient outcomes, enhance healthcare access, and manage general practice workload. Building relationships with pharmacy professionals across the PCN and wider health system will be key.

Working with the GP Pharmacy Clinical Lead, they will help develop and implement updated clinical strategies and lead on performance and operational goals within the Pharmacy Service. The role involves managing pharmacy staff, collaborating with healthcare providers, and ensuring regulatory compliance to enhance patient care, medication management, and resource efficiency.

CLINICAL RESPONSBILITIES

Long Term Condition Clinics- Review patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma, depression and frailty). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Instigate changes and liaise with GP.

Clinical Medication Review- Undertake clinical medication reviews with patients, instigate changes and liaise with GP. This would include reviews of those patients on the Serious Mental Health Register.

Care Home Medication Reviews- Undertake clinical medication reviews with patients and instigate changes and liaise with GP. Work with care home staff to improve safety of medicines ordering and administration to comply with the Enhanced Health in Care Homes specification in the DES.

Domiciliary clinical medication review- Undertake clinical medication reviews with patients, instigate changes and liaise with GP. Attend and refer patients to multidisciplinary case conferences.

Management of common, minor, self-limiting ailments: Management of common, minor, self-limiting ailments presenting to general practice. Managing a caseload of patients with common, minor, self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to other healthcare professionals where appropriate.

Telephone medicines support- Provide a telephone helpline for patients with questions or queries. Medicine information to practice staff and patients: Answer relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams e.g. community pharmacy, and patients with queries about medicines via triage and E Consult. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.

Management of medicines at discharge from hospital- Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients e.g. those with medicine compliance aids or those in care homes.

Repeat Prescribing- Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review. Ensure patients have appropriate monitoring tests in place when required.

Service Development- Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components, e.g. advice on treatment pathways and patient information leaflets.

Information management- Analyse, interpret and present medicines data to highlight issues and risks to support decision-making.

Medicines Safety- Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Education and Training- Provide education and training to primary healthcare team on therapeutics and medicines optimisation.

Care Quality Commission- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

Public health- To support public health campaigns. To provide specialist knowledge on all public health programmes available to the general public
