enthusiastic, motivated experienced general practice prescribing pharmacist who
is committed to developing knowledge and skills to improve patient care in
general practice.
Main duties of the job
The post holder will work
as part of a multi-disciplinary team and provide clinical pharmacy leadership
for the practice, taking responsibility for, and not limited to, Structured
Medication Reviews, areas of chronic disease management, medicines optimisation,
quality improvement, the quality and outcomes framework and other enhanced
services.The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. The post holder will provide clinical leadership on medicines optimisation and quality improvement and be responsible for supporting the GPs in implementing effective medicine management within the practice, including undertaking searches and audits identifying areas for improvement, initiating and managing change, and monitoring budgets.
This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent services within general practice.

We are fully computerised and a leading practice for on-line records access, with currently 13,000 patients across 2 sites in Hyde & Denton.
We work collaboratively with the other 7 Practices that are part of the Hyde PCN and provide health & social care for patients.

Competently manage repeat prescribing systems, deal with acute prescription requests and medicines reconciliation on transfer of care, if necessary putting in place systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices.
The post holder will provide support to practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage GP workload.
The role is pivotal to improving quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice.
