Job summary

From 1st April 2025, PCCO PCN will be providing our own Team Up service, providing home visits, chronic disease management and holistic support to our complex and vulnerable patients.

We are looking for a Community GP to provide day-to-day clinical leadership for this for one or two days per week (09:00 - 18:00).

The GP will collaborate with the PCN Service Clinical Lead and Operational Manager to develop and deliver a safe, effective service, creating new clinical pathways and working with local stakeholders to promote integrated care for the PCN population.

This role will be shared with other GPs, who may have a portfolio at Lister House Surgery. We believe this could be an excellent opportunity for professionals development and to contribute to helping those with the highest needs.

The contract is currently one year, with the hope of extension moving forwards.

Main duties of the job

  • Lead a multi-disciplinary, multi-agency team to deliver high-quality Primary Care services at home, in care homes, and for complex patients.
  • Mentor and support clinical decision-making.
  • Provide medical assessments and management.
  • Run MDT meetings for complex patients.
  • Conduct senior clinical triage for Acute Home Visiting.
  • Educate and mentor team members, especially those in advanced roles.
  • Contribute to quality improvement activities.
  • Build relationships with social care, ambulance, community, and mental health clinicians.
  • Support complex needs services, care homes, and palliative patients.

About us

We are GP teaching practice with a dedicated team committed providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues for Derby City and the surrounding area.

We have GP Partners, associate partners, salaried/locum GPs along with ANP's, FCP's, Mental Health Team, Health and Well Being staff as well as social prescribers. In addition to these we also have 23 staff in the Nursing Team which includes 5 Health Care Assistants and 1 trainee Nursing Associate. We deliver high quality care to a 45 500-patient population. The clinical team is supported by an extensive administrative and management team.

  • Recognised qualification in EOL
  • Recognised qualification in medical education/clinical supervision
  • Evidence of leadership development

Experience

  • Experience of working in the community

  • Experience and evidence of an interest in care of the Elderly and Palliative care
  • Understanding of adult safeguarding and Deprivation of liberty procedures
