Job summary
The PCN Pharmacy Technician will work as part of a MDT and pharmacy team comprising of other clinical pharmacists, pharmacy technicians and pharmacy administrators. Pharmacy Technicians will jointly support and mentor the pharmacy administrators with medication and patient related queries. They will be a point of contact day-to-day for the Pharmacy Administrators. PCCO PCN has a a diverse younger than average patient population with higher levels of deprivation linked with obesity, smoking, mental health, and cardiovascular disease. PCN main practice branch is centred in Normaton with a multicultural population adding challenges of language barriers, health inequalities, and patient understanding of health.
Main duties of the job
The post holder must be passionate about patient and drug safety, prescribing guidelines and in supporting patients with their medication queries. The Pharmacy Technician will manage hospital discharges and clinic letters transcribing when appropriate and following local guidelines on prescribing. They will support with medication reconciliation, liaising with care homes with mediation queries, and supporting the wider practice team with drug related queries. The Pharmacy Technician will with the Clinical Pharmacists ensure the practice has drug safety and drug monitoring processes. The Pharmacy Technician will provide education to the wider practice team on the latest local ICB Medicines Management Team updates and with out-of-stock queries.
They will contribute to the PCN in ensuring that targets around the PCN DES, IIF and practice QoF, drug safety and monitoring searches are met.

- To work as part of a multi-disciplinary team providing support to the practice with regards to prescriptions and medication queries promoting safe, efficient, and cost-effective prescribing.
- To help support the repeat prescriptions system including to reduce wasteful prescribing and inappropriate polypharmacy.
- To support the practice with managing out of stock medication queries and to advise on the sourcing and safe management of medicines as appropriate
- To act as a link between the practice and local community and/or hospital pharmacy services to ensure medication related queries are dealt with in a timely manner
- To undertake medicines reconciliation e.g., hospital discharges, hospital letters
- To support with drug safety e.g., MHRA alerts and drug monitoring
- To participate in medication audits
- To work alongside our care homes providing medicines support
- Support the Lead Clinical Pharmacist in ensuring Pharmacy Technician representation at local ICB Medicine Management Team meetings and other PCN/Practice Meetings
- Work with primary care professionals and patients to implement NICE and other evidence-based guidelines
- To support the Clinical Pharmacist to provide medication reviews, patient information leaflets, medicine awareness
- Actively participate in the development and updating of Practice Protocols/guidelines and procedures using evidence-based research
- Computer data entry, processing and recording information in accordance with practice procedures
- Provide health promotion and education
- Support the Lead Clinical Pharmacist in mentoring/training the pharmacy administrators
- Supporting the wider practice clinical team with queries, training and education including developing induction programmes for new clinical staff
- Teaching and educating patients and their families by offering information which enables them to understand their illness and its management with medicines. This may be provided over the telephone or patient facing
