PCCO PCN (Lister House) is a forward-thinking primary care network keen to embrace the opportunity to work with Allied Health Professionals to enhance the service we offer to our patients. We are looking to employ a PCN Clinical Pharmacist to join our growing multi-disciplinary team.
The practice is committed to supporting the successful candidate in a career in general practice where you will have opportunities to enhance your skills in a primary care setting with the support of our experienced pharmacy team. You will be required to enrol on the CPPE Primary Care Pharmacy Education Pathway and undertake training to gain the independent prescribing qualification if you do not have this already.
Main duties of the job
The role is
- To work as part of a multi-disciplinary team in a patient-facing role taking responsibility for areas of chronic disease management within the practice.
- To undertake structured medication reviews to proactively manage patients with complex poly-pharmacy, especially for older people and those with multiple co-morbidities.
- To provide support to general practice staff with regards to prescription and medication queries.
- To help support the repeat prescriptions system and deal with acute prescription requests.
- To undertake medicines reconciliation on transfer of care and contribute to the development of systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).
- To proactively work alongside the wider PCN, MSK and Mental Health multidisciplinary teams, on projects such as tackling health inequalities, low prevalence for CVD and high levels of opiates.
The role is pivotal to improving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within general practice.
