Job summary

We are looking for an enthusiastic Experienced Practice Nurse to join our expanding team of nurses at our friendly practice to cover holidays and ad hoc clinical campaigns.

Lister House consists of 4 sites across Derby and you may be required to work at any of these sites depending on business needs.

Main duties of the job

Working as part of the practice multidisciplinary team, the post holder will ensure skilled nursing services are delivered effectively to the practice patient population. The practice nurse will be responsible for delivering health promotion, chronic disease management, health prevention, and screening for women, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure

About us

We are GP teaching practice with a dedicated team committed providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues for Derby City and the surrounding area.

We have GP Partners, associate partners, salaried/locum GPs along with ANP's, FCP's, PA's, Mental Health Team, health and wellbeing staff as well as social prescribers. In addition to these we also have 23 staff in the Nursing Team which includes 5 Health Care Assistants and 1 trainee Nursing Associate. We deliver high quality care to a 45 500-patient population. The clinical team is supported by an extensive administrative and management team.

Job responsibilities

1. General

To assess patients' health needs and implement nursing interventions to meet their needs

To manage the care and treatment of patients with stable or long-term conditions within agreed protocols

To maintain high standards of infection control at all times, protecting patients, self and colleagues

To actively engage patients and carers in the management of their health, promoting health and wellbeing

To refer patients to other members of the practice clinical team in line with agreed procedures

To maintain accurate records of all clinical activity in line with practice protocols and NMC standards

To assist in specialist clinics within the practice protocols

To contribute to clinical governance activities within the team including audit and evaluation

To support other members of the practice team in maintaining health, safety and security, identifying and assessing potential risks and contributing to management

To limit your actions to those which you feel competent to undertake. If you have any doubt about your competence during the course of your duties, you must immediately speak to your line manager

To develop the role by using evidence-based practice and continuously improve self-knowledge following continuing professional development guidelines

2. Clinical Duties

Treatment Room Duties

Giving injections to the practice population according to protocols

Ensure adequate stock levels in stock room, treatment rooms, fridges and cupboards

Inputting of information onto SystmOne

Ability / confidence to work alone in the treatment room, seeking advice from colleagues / GPs when necessary

Some pharmacological knowledge required, especially training to wound care / infection

Wound care be confident in wound assessment, be able to identify signs of infection and use suitable dressings after initial assessment. Have a broad knowledge of wound types and methods of management; i.e. burns lacerations, sebaceous cysts/drainage and insertion of wick dressings, post-operative wound care, and suture and clip removal, minor skin injuries; i.e. grazes and bruises. Refer patients to medical colleagues for a review when necessary

Leg ulcer dressings and management knowledge of ulcer dressings, ability to assist with these dressings in the absence of a specialist nurse.

Women's Health

To advise patients on the national and local cervical cytology and mammography screening programmes

To advise on breast self-awareness

Cervical cytology attend relevant recognized course for cervical cytology and possess PIN for LBC

Undertake vaginal swabs and discuss this with patients, offer sexual health advice and give relevant contact numbers / information as required

Vaccinations and Immunisations

To administer injections under an individualised prescription or Patient Group Directive (PGD)

To ensure safe storage, rotation and disposal of vaccines

The provide an adult and childhood immunization service

To maintain a call and recall system for patients

To work to PGD for prescription only medicines

Patient Screening

To undertake opportunistic screening, including blood pressure checks, height/weight and BMI calculations, urinalysis and smoking status

Infection Control

To apply infection control measures within the practice including:

1. Universal hygiene precautions

2. Collection and handling of laboratory specimens

3. Segregation and disposal of waste materials

4. Decontamination of instruments and clinical equipment

5. Reporting on and treatment of sharps injuries

6. Dealing with blood and bodily fluid spillages

Emergency Management

To be competent in anaphylaxis and resuscitation techniques

Child Health/Protection

To recognise the signs, symptoms and categories of child abuse

To understand the importance of recognition, observation, documentation and communication channels

To be aware of statutory child health procedures and statutory local guidance and basic legal issues

To be aware of other practitioners and agencies roles
