Job summary
The Advanced Nurse Practitioner (ANP) at Chadderton Medical Practice plays a vital role in delivering high-quality, patient-centred care. As a highly skilled and experienced healthcare professional, the ANP has demonstrated advanced clinical competency and expertise in diagnosing, treating, and managing a broad range of health conditions. Operating autonomously, the ANP assesses patients, makes independent clinical decisions, and initiates appropriate interventions, including prescribing medications and ordering diagnostic tests. ANPs at Chadderton Medical Practice work closely with patients, developing long-term relationships built on trust and collaboration. By offering expert care without the need for GP referral, the ANP ensures that patients receive timely treatment, reducing delays and enhancing overall healthcare access. Additionally, the ANP evaluates treatment outcomes and refers patients to specialists or other healthcare services when necessary, ensuring comprehensive care. Main duties of the job
Develop and lead health promotion and wellbeing programmes.
Manage acute and chronic medical conditions, creating and evaluating treatment plans.
Provide advanced nursing care, wound care, and specialist clinics (e.g., contraception, ear syringing, travel medicine).
Request and interpret pathology tests, and maintain accurate clinical records.
Lead chronic disease management, including maintaining disease registers.
Prescribe medications, deliver immunisation programmes, and manage mental health needs.
Ensure safeguarding, supervise staff, and contribute to practice targets.
Participate in clinical meetings and respond to clinical emergencies.
