Job summary

Blacketts Medical Practice in Darlington is looking for an enthusiastic, forward-thinking Clinical Pharmacist to join our team.

Main duties of the job

The post holder will carry out a mixture of face to face and telephone reviews with patients as well as supporting the repeat prescription system, dealing with acute prescription requests and medicines reconciliation.

About us

We are a successful practice with a list size of around 11,500 patients. We have 5 partners and 2 salaried doctors, an excellent nursing team and a stable, friendly and efficient administration team. We score highly for QOF and were rated "good" in our last CQC inspection. We are a SystmOne practice.

We have spacious purpose-built premises in the centre of Darlington, which also houses private physiotherapy and chiropody services. Our space means that we are able to provide rooms for social prescribers, pharmacy technicians and mental health workers, which enables us to work closely with them to provide excellent patient care.

Job description

Job responsibilities

· Work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

· Design, develop and implement computer searches to identify cohorts of patients at high risk of harm from medicines.

· Work with patients and the primary care team to minimise risks through medicines optimisation.

· Review the use of medicines most commonly associated with unplanned hospital admissions and re-admissions through audit and individual patient reviews.

· Review patient requests for repeat prescriptions, review medicines reaching review dates.

· Ensure patients are booked in for necessary monitoring tests where required and review and file test results.

· Provide a telephone help line for patients with questions, queries and concerns about their medicines.

· Hold clinics for patients requiring face to face medication reviews.

· Undertake risk assessment and management and ensure compliance with medicines legislation.

· Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

· Ensure patients are switched to the most cost effective medication.

· Answer all medicine related enquiries from GPs, other staff and patients.

· be responsible for maintaining registration as a pharmacist and complying with appropriate professional codes.

· keep up to date with medical / therapeutic evidence and opinion, local and national services, legislation and policy developments.

· attend local, regional and national meetings and training courses of relevance.

· comply with all organisational and statutory requirements (e.g. health and safety, equal opportunities and diversity, confidentiality and clinical governance).

Essential

  • Completion of degree in Pharmacy and registration with the General Pharmaceutical Council.
  • Independent prescriber or willing to work towards qualification.

Desirable

  • Completed or working towards CPPE primary care pharmacy education pathway.

Knowledge and Skills

  • Excellent communication skills.
  • Excellent clinical knowledge and ability to apply to practice.
  • Ability to work without supervision.
