Job summary

The receptionist / care navigator's role is to support the practice clinical team by signposting patients to the appropriate healthcare professional or service, and to undertake reception duties supporting the administrative team in delivering a polite and professional service to the entitled patient population.

Main duties of the job

The main duties of the role are to be the first port of call for patients, professionals and other parties contacting the surgery, whether in person, via telephone or other means (including email). Whether booking appointments, processing prescriptions or actioning other clinical or adminsitrative tasks, a high quality, patient-centred manner is crucial.

Job description

Job responsibilities

The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:

a. Provide high quality, patient-centred service to all patients

b. Effectively and accurately process, in a timely manner, all personal, telephone and e-requests from patients and third parties

c. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition

d. Answer incoming phone calls, transferring calls or dealing with the callers’ requests appropriately

e. Process patient requests for appointments

f. Process repeat prescription requests

g. Initiate contact with and respond to requests from patients, team members and external agencies

h. Enter read-code data on Vision

i. Photocopy documentation as required

j. Data entry of new and temporary registrations and relevant patient information as required

k. Input data into the patients’ healthcare records as necessary

l. Direct requests for information, e.g. SAR, insurance/solicitors’ letters and DVLA forms, to the administrative team

m. Manage all queries as necessary in an efficient manner

n. Carry out system searches as requested

o. Maintain a clean, tidy, effective working area at all times

p. Monitor and maintain the reception area and noticeboards

q. Support all clinical staff with general tasks as requested

Secondary responsibilities

In addition to the primary responsibilities, the Care Navigator may be requested to:

a. Participate in practice audit as directed by the audit lead

b. Support administrative staff, providing cover during staff absences

c. Action incoming faxes when necessary

d. Scan patient-related documentation and attach scanned documents to patients’ healthcare records

e. Complete opening and closing procedures in accordance with the duty rota

f. As required, support the prescribing clerk(s) in the management of repeat prescriptions, ensuring that they are processed accurately and efficiently

g. Order and monitor stationery supplies
