Job Summary:

This is an opportunity to be a key member of the growing Avon Valley PCN Team, as we look to develop our Home Visiting Service, and develop a new model of healthcare delivery as per the NHS Long Term Plan (2019). We are looking for a minimum of 2 Primary Care Paramedics to work across the 4 practices which make up our network, with a patient population of 42,000.

Main duties of the job

The primary role of our Community Paramedic team will be to support practices by providing an on the day emergency Home Visiting Service, involving the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions.

With further service development, secondary roles may include Anticipatory Care (advanced Care Planning/Frailty) and the management of chronic disease and long terms conditions.

The post holders will work closely with the GP teams, receiving clinical support but allowing for professional independence, and would be required to work within the Advanced/Emergency Care Practitioners level of competency.

About us

Avon Valley PCN is an innovative, friendly and forward thinking group of training practices situated in the New Forest around Ringwood, and encompasses four practices; Fordingbridge Surgery, Ringwood Medical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery in Bransgore.

As practices we have been working together for some time and have an excellent and established working relationship. Together we aim to build robust services across our network to improve patient care and are keen to bring new members into our multi-disciplinary team to achieve these goals.

Clinical

To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs in patients home, care home or practice base.

To support and continue the development of an on the day emergency Home Visiting service for the network.

To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

To evaluate clinical information from examination, history taking and initiate appropriate treatment and/or referral.

To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To apply clinical guidelines (e.g. NICE and local guidelines) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.

To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.

Communication

To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.

To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.

To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission.

To take an active role in maintaining expert communication with all patients, carers and relatives which meets their individual needs.

To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.

To identify risks to service provision and highlight them to the Clinical Lead.

To work within the requirements of Data Protection legislation as it applies to patient records.

To maintain current professional registration.

To act at all times in such a manner as to promote confidence and public trust and to uphold the reputation of Avon Valley PCN, the practices represented by the Network and their own profession.

Essential

  • Educated to degree/diploma level in Paramedicine or equivalent experience.
  • Registered with the Health and Care Professions Council (HCPC)
  • Has completed their two year consolidation of learning period as a newly qualified paramedic.
  • Has a further 3 years as a Band 6 (or equivalent) paramedic
  • Within six months of commencement to complete the clinical pillar competencies of paramedics, FCP/AP road map to practice.

Desirable

  • Currently working towards a masters level or equivalent capability in paramedic areas of practice.

Experience

  • Advanced autonomous clinical skills and a broad, in-depth theoretical knowledge and paramedic practice base.
  • Excellent history taking, consultation and examination skills to diagnose and manage illness and injury.
