Job summary
We are looking to welcome an experienced Clinical Pharmacist
to join our dynamic and supportive team at Humberstone Medical Centre. This
vacancy is up to 15 hours per week, salary depending on experience.
Main duties of the job
Were looking for enthusiastic and forward-thinking
pharmacist to complement the team. The
successful candidate will hold clinics for patients requiring medication
reviews, manage prescription queries and be involved in long term condition
management. We welcome applicants with or without GP experience. If you have
the right skills and qualities, we can provide on-going support to develop
professionally.

Humberstone Medical Centre is a thriving and innovative
practice providing care in the Northeast area of Leicester to 12,000 patients. We
are a friendly and experienced team who work collaboratively across clinical
and administrative roles.
We have a commitment to high quality evidence-based care
with a strong ethos of providing and enabling training and support to all our
staff, both clinical and administrative.
We are a large and friendly team including 5 GP Partners, 3
Salaried Drs, 2 Pharmacists, 1 Pharmacy technician, 3 Nurses, 2 HCAs, an FCP, a
Social Prescriber, care coordinators and administrators.
We are a teaching practice and take GP Registrars and
Medical students on rotation.
We are part of Salutem Primary Care Network and take a lead
role in it with the previous and current Clinical Director both being partners
in the practice. Salutem PCN has an excellent reputation for collaborative
working in multiple projects and seeking to maintain high standards of patient
care in all 5 practices within it.

Hold face to face and telephone clinics for

Undertake structured medication reviews with
multi morbidity and polypharmacy and implement changes whilst ordering relevant
monitoring.

Essential
- Independent Prescribing qualification or working towards this qualification
- Current registration with General Pharmaceutical Council. A GPhC registered pharmacist
- Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice
- Excellent communication and interpersonal skills
- Therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
Desirable
- Minor ailments certification
- Membership of the Royal Pharmaceutical Society
- Medicines management qualification

- Experience of dealing with vulnerable patients
- Ability to promote best practice regarding all pharmaceutical matters
- Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face
- Excellent interpersonal, influencing and negotiation skills organisation skills with the ability to constructively challenge the views and practices of managers and clinicians. Ability to use own initiative, discretion and sensitivity
- Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
