We are looking for an Advanced Nurse Practitioner to join
our Nurse- led, innovative, friendly, supportive General Practice in Hanwell W7.
We are based in a modernised purpose built premises. We have been awarded a CQC
outstanding status and General Practice award Nursing team of the year.
This is an exciting opportunity for the right person to join
our team to deliver exceptional services to our patients. To work with
multidisciplinary team with support of Nurse Practitioner Director, Nursing team,
GPs and administrative team.
Main duties of the job
Lead on modernising patient pathways, supporting autonomous clinical decision-making, and enhancing care delivery across a progressive, community-focused practice.

- To provide Nurse Practitioner services to patients of the practice, including diagnosis, treatment, screening and advice within the professional competence of the post holder.
- To provide clinical leadership including acting on Clinical Governance issues, such as urgent and routine results, letters and on instructions from outside agencies such as hospitals and consultants.
- To work independently in the surgery in the absence of a GP, possessing the initiative and confidence to do so.
- To work as part of the Primary Healthcare Team, sharing decision-making and information where appropriate.
